Waiver/Release

attend, or my dog; that despite all the dogs appearing healthy and being
handled with the greatest amount of care and foresight, dogs are not always
predictable and the unexpected may occur.
I hereby waive and release Paw In Hand Dog Training, its employees,
volunteers, owners and agents from any and all claims while on the grounds
or surrounding areas thereto, and resulting from participation in training at
Paw In Hand Dog Training. Including specifically, but without limitations, any
injury or damage resulting from the action of any dog, including my own.
I understand and agree that results of training may vary due to many factors
including, but not limited to the breed or mix of the dog, prior experiences
and the amount of time spent practicing the training.
I further understand that participants and guests in classes are expected to
abide by the rules and policies set forth by Paw In Hand Dog Training and
agree to abide by those rules and policies.

Vaccinations
Dogs and puppies are required to be vaccinated against rabies as per
NC State law. Vaccinations against distemper, parvovirus, hepatitis,
parainfluenza and bordatella should be administered on the schedule
recommended by your veterinarian. Please mail a copy of proof of
vaccination or titer results or bring them to the first class. No puppy or dog
will be allowed in class with out their vaccination record.
