AFFIDAVIT TO RETURN HOME

I, BLANCA MANALAD PERALTA, of legal age, Filipino citizen, single and

presently residing at 24 Interior Milagrosa Street, Bagong Bario, Caloocan City, after
having been duly sworn to in accordance with law, hereby depose and say:
1. That I am currently working as an Account Manager in Hordex Enterprises
located at Quezon City since January 23, 2017 up to present with an Annual Salary of
Php 196,776.00. Also, I work as a part-time Health Counselor for Caritas Health Shield
and as Accredited Real Estate Salesperson for DMCI Homes Inc.
2. That I intend to go and visit Canada to witness my mother’s 70 th Birthday on
March 23, 2018 which will also serves as our family reunion. Belinda Manalad Peralta,
our eldest sister invited me and supported my expenses including the round trip plane
tickets and related expenses for the entire duration of my stay at her residence located at
3457 Old Orchard, Missisuaga, Canada.
3. That the purpose of executing this affidavit is to show good faith and this is done
for the purpose of taking a tourist visa and that I will be returning to the Philippines to
resume my work obligations, and my financial responsibilities in the Philippines such as
car loans, Philam Insurance and condominium monthly payments. Reasons that I need to
go back immediately after my visit and attend the abovementioned duties.
4. That I am executing this affidavit to attest to the truth of the foregoing facts and
for whatever legal purposes this may serve.

I am Blanca Manalad Peralta, Filipino citizen, born on October 5, 1977 and
currently residing at 24 Interior Milagrosa Street, Bagong Barrio, Caloocan City, with a
Passport Number ECA4855219. I intend to obtain a tourist visa for my travel and visit to
Ontario, Canada this March 17 to April 2, 2018.
I am currently working as an Account Manager in Hordex Enterprises located at
Quezon City since January 23, 2017 up to present with an Annual Salary Php 197,
776.00. I am also a Health Counselor for Caritas Health Shield as my part-time work an
also as accredited Real Estate Salesperson for DMCI Homes Inc.
In line with this, my purpose for this visit is to witness my mother’s 70 th Birthday on
March 23, 2018 which will also serves as our family reunion. My eldest sister belinda
Manalad Peralta invited me and supported my expenses including the round trip tickets
and other related expenses for the entire duration of my stay at her residence located at
3457 Old Orchard, Mississauaga, Canada.
I love to travel, and to be able to visit Canada especially the famous Niagara Falls
will be a big check as one of my bucket list. I have travelled and visited Asian Countries
such as Bali, Indonesia and Kuala Lumpur, Malaysia in 2011 respectively and Singapore
in 2013. Also in European Country such as Brussels Belgium in 2012.
In this regard, I still have my financial obligations in the Philippines (car loans,
Philam Insurance and Condominium Monthly payments), reasons for me to get back
immediately after my visit.
