AFFIDAVIT

I, PO3 HAZEL G. GARCIA, Filipino, of legal age, married and the assigned
property custodian and patrol member, currently stationed at the Leganes
Municipality Police Station, Leganes Iloilo after having sworn to in accordance
with law, hereby depose and state:

That on _________________ at around 1:00 o’ clock in the morning a certain

Ronald Regala, one of the crew members of SS Internet Station located at the
town proper of Leganes, Iloilo went to the police station and reported about a
stabbing incident positively identifying a certain John Ray Agregado as the one
who stabbed PJ NAZARENO;
 That upon the order of our chief commanding officer PSI Rogelio Ortigas, I
together with PO1 Jee Fabregas and PO2 Ranny Onelongod immediately
responded to the scene of the incident while SPO2 Alberto H. Sarmiento drove
Michael Hopilos and Gabriel Ventura to West Visayas State University Hospital at
Jaro, Iloilo City. I was the one who drove the Mobile Patrol from the Police
Station to the scene of the incident until we conducted hot pursuit operation and
back to the police station;

That when we arrived, we conducted series of interviews to the people around

the area who actually witnesses the incident; and that they positively identified
John Ray Agregado as the culprit and the person who stabbed PJ Nazareno. Said
testimonies were even corroborated by _____________, friend of the victim, PJ
Nazareno who, among others actually witnessed the gruesome stabbing incident;

That after the standard inquiries and investigations, the entire team proceeded
to conduct Hot Pursuit Operation, still per command of PSI Rogelio Regala;

That however, despite due efforts and diligence to search for said John Ray
Agregado, we were unsuccessful in apprehending him. At first we went to his
house, then to the house of his Aunt and to other places where he could possibly
be, BUT TO NO AVAIL;

That after the failed hot pursuit operation, we went back to the police station
and have the incident blottered and rendered our spot report;

That I am executing this affidavit to attest to the truth of the foregoing facts.
