AFFIDAVIT OF ACKNOWLEDGEMENT

I, JUAN DELA CRUZ, of legal age, married, Filipino citizen and a

resident of Zamboanga del Norte, after having been duly sworn to in
accordance with law, do hereby acknowledge that:

1. I am the biological father of PEDRO HERRERA;

2. My child, PEDRO HERRERA, was born on July 31, 2005 at Guildo

Memorial City Hospital, Barangay Cosin, Iligan City;

3. The fact of birth of my child was duly registered at the City Civil
Registrar of Iligan City;

4. Being the father of PEDRO HERRERA, I hereby acknowledge and

recognize PEDRO HERRERA as my child and legal heir;

5. I am allowing my child, PEDRO HERRERA, to use my surname as

his surname in his Certificate of Live Birth;

6. As such, I am executing this Affidavit to attest to the foregoing facts

and for whatever legal purposes this may serve.
