The CKHS Booster Club is a volunteer organization that promotes the extracurricular activities of
CKHS students. The Booster Club provides various scholarships up to $2,000 to seniors that have
chosen post-secondary education at a college, university, or technical/vocational school.

chosen that career path, and why you would be a good choice for this scholarship.

LETTER OF RECOMMENDATION: *Include one letter of recommendation from someone who

knows about your Community Involvement. For example this could be an employer, minister,
teacher, or supervisor. Please do not include a letter from a school counselor.
