2019 COA SINGING CONTEST

REGISTRATION FORM

Registration Guidelines:
1. Kindly read the instructions carefully before filling-out the Registration Form. Fill-out all the
necessary information, especially the contact details.
2. All registration forms for the inter-regional level must be submitted to the respective regional
cluster singing competition committees at least five (5) working days before the competition.
3. For the national competition, the representative/s from each regional cluster must submit this
form to the Committee on Singing Contest Secretariat (at __________@gmail.com) on or before
April 13, 2019, not later than 5:00 P.M.

5. Early submission of registration forms is highly encouraged. Incomplete submission will
automatically result in disqualification.
