CONTRACT OF SERVICES

KNOW ALL MEN BY THESE PRESENTS:

This contract is made and entered into by and between:

The Local Government Unit of Kibawe, Bukidnon with office address at Kibawe, Bukidnon
herein represented by Honorable MINERVA C. CASINABE, Municipal Mayor, hereinafter referred to
as the “FIRST PARTY”.

EMELYN M. JAKOSALEM , of legal age and postal address at Romagook, Kibawe, Kibawe,
Bukidnon, herein referred to as “SECOND PARTY”;

W I T N E S S E T H:

Parties hereby agreed on the following terms and conditions:

1. That the First Party is in need of the services of the Second Party to perform work not
performed by the regular employees/personnel;

2. That the Second Party has signified his intention to which the First Party has accepted to
provide the services needed by the latter;

3. That the Second Party possesses the required educational qualification, profession, skills and
experiences to perform the job as described herein;

4. That view hereof, the Second Party is hereby contracted as Clerk in the Municipal Budget
Office, for the period starting January 3, 2019 to June 30, 2019 provided that renewal
of the contract is at the option of either Party but in no case shall exceed one year period;

5. That the First Party hereby agrees to pay the honorarium to the Second Party in the amount of
FORTY FIVE THOUSAND PESOS (P48,000.00) only to be paid EIGHT THOUSAND
PESOS (P8,000.00) per month for the compensation of the job accomplished or
6. P 4,000.00 every quincena; chargeable against the appropriation of Other Services in the
Municipal Budget Office .

7. That as Project Engineer Assistant , the Second Party shall perform the following:

 Assist in the review of BarangayBudgets;.

 Assist in the Barangay Transactions and Reports;
 Does other work tasks.

IN WITNESS WHEREOF, both parties have hereunto set their hands this ____ day of ________
