MEMORANDUM OF AGREEMENT
This contract made and entered into this _____ day of Oct.__,2017 at City of San Fernando, La Union
Philippines by and between:

The Personal Collection Direct Selling Inc, a direct selling company, duly organized and existing
under the Philippine Laws, with principal office at Triumph Bldg., Quezon Avenue, Quezon City that
branches at City of San Fernando, La Union represented herein by the Managing Director of La Union
Branch, Ms. Irene Rose Garcia and Ms. Elizabeth Villarubia Branch Collection Officer.

AND

_________________________ a resident of Barangay Payocpoc Norte Weste Bauang, La Union, of legal

age, Filipino Citizen, herein after referred to as the SUBSCRIBER.

WITNESSETH

Whereas, the parties hereto in consideration of the mutual covenants provided, mutually agree

1. That, for and in consideration of the PA-RAFFLE BONANZA, that will be held on December
29,2017, 2pm at Brgy. Payocpoc Norte Weste Multi- Purpose Hall subjects to the terms and
conditions:

That the prizes that will be given are as follows:

1st Prize __________________________________ P 10,000.00

2 Prize __________________________________ P 8.000.00

3 Prize __________________________________ P 5,000.00
4th Prize __________________________________ P 3,000.00

In addition to, there will be 10 Consolation Prizes of grocery items worth P300.00.

2. That, the Proceeds of this Pa-Raffle Bonanza (P50,000.00) will be given to the said Barangay as
an Additional Fund for its’ Improvement.
 3. That, the following details below is CONSIDERED SOLD.
250,000 pcs.tickets at P10,00 each, P 250.00 per booklet, for a total of P 250,000.00,inclusive of
150,000.00 worth of FWA’s products (Personal Collection and Winnerslife)

4. That, A. The first payment (not less than P 50,000.00) will be given on November 18,2017
B. The second payment (not less than (100,000,00) will be given on December 18,2017
C. The Full payment or remaining balance will be given on or before December 27,2017.

5.Hence,Remittance of payments are as follows:
