AFFIDAVIT OF DISCREPANCY IN NAME

I, ANGELICA PINTOR OROSCO, of legal age, single, Filipino, and a resident of Upper Panabang,
Apas, Cebu City, Cebu, Philippines after having been duly sworn in accordance with law, hereby depose
and state: That-

1. That my name is Angelica Pintor Orosco and I solemnly declare that I am the same individual
as stated in the following documents:

 National Statistics Office, Office of the Civil Registrar document birth certificate with Local
Civil Registry No. 90-1887. A copy is attached herein as Annex “A”;
 COMELEC I.D. No. VIN:2217-0055A-H2490APO20000 issued in Cebu City. A copy is
attached herein as Annex “B”;

2. I am processing documents for the purpose of securing a marriage license. Upon checking the
entries in my certificate of live birth as mentioned above with Local Civil Registry No. 90-1887, I noticed
that the spelling of the full name of my biological father is incorrectly spelled as “Guillermo Barret Orosco”
instead of the correct spelling of his full name “Guillermo Barrit Orosco”.

3. I am affirming the fact that the correct spelling of my biological father’s name is “Guillermo Barrit
Orosco” as shown in herein documents, to wit:

 Certificate of Birth & Baptism, issued by the Parish of St. Francis Xavier, Alegria, Cebu,
Philippines issued on December 30, 1979. A copy is attached herein as Annex “C”;
 Philippine Veteran’s Affairs Office I.D. No. A-17-3354, issued on 08 September 2017 with
expiry date 08 September 2019.
 Armed Forces of the Philippines I.D. No. R09-E-44192, issued on 25 April 2009.
 BIR TIN I.D. No. 124-229-866-000, issued on 01/07/2003.
Copies of the Identification Cards are shown in the document attached herein as Annex
“D”.

4. The name with the incorrect spelling “Guillermo Barret Orosco” and the correctly spelled name
“Guillermo Barrit Orosco” refer to one and the same person, “Guillermo Barrit Orosco” who is my biological
father;

5. I am making this affidavit to clear any confusion as to identity of my biological father and to
affirm the correct spelling of his full name.

6. I am making this affidavit to declare the inconsistencies with the names indicated on said
documents and I make this solemn declaration under oath, conscientiously believing it to be true and
correct.

7. I am executing this affidavit to attest to the foregoing facts and for any legal purpose it may
serve.
