Dear Ms. Dela Cruz:

Please explain in writing within five (5) working days from your receipt of this
Memo why no disciplinary action shall be imposed upon you for the following
violations of the Company’s Code of Ethics and Discipline:

If proven, these acts or omissions may constitute unauthorized acquisition, use


or disposal of company property/assets, including materials and supplies, and
fraud or willful breach of the trust reposed in you, and loss of trust and
confidence. Please note that these are sufficient grounds for disciplinary action,
including termination from employment, under Article 297 of the Labor Code and
Company Code of Ethics and Discipline.

Your failure to reply within the given period shall be taken as a waiver of your
right to give your explanation and in which case, management shall decide on
this manner based on evidence on hand.

Furthermore, your position as System Finance Officer is a position of trust as you


are privy and have access to vital and confidential information of the company.
In view of this, you are hereby placed under preventive suspension for 30 days
effective upon receipt of this notice.
