Data Gathering Procedure
General Information
We are being commissioned to perform a structural investigation work for VMMV SICU
BUILDING project through DIR. DOMINADOR M. CHONG JR. MD, FPCS, FACS (VMMC
Director). This document explains in detail the methodology covered in our Data Gathering Phase.
The following are the project details covered in our investigation works:
Sequence of completion for each procedure, including actual locations, is subject to changes and
verification. Final location of testing samples will be determined only during the project duration.
Actual physical measurement, rebar scanning and rebound hammer testing will be performed by
our team; while testing of Concrete core sample (as needed), including minor chipping and drilling
works, shall be performed by a third party contractor, which is under and witnessed by our team.
Restoration for any damages on structural members caused/required by the specified test (i.e.
chipping, drilling, etc.) shall be the responsibility of our team.; while architectural elements (i.e.
ceiling claddings, fixtures, etc.) that need to be dismantled/cut and restored, including accessibility
and manpower assistance, shall be covered by the owner/others (and shall not part of our scope of
work.)

A.2. Procedure
2.) Establishing Building Gridlines and Story heights, and slab thickness
- In the event that critical structural members are covered by architectural cladding, the
concrete member will be stripped down of all obstructions until the necessary
dimensions are noted
B. Rebar Scanning
B.1. Description
Determining/verifying the on-site rebar schedule of affected structural elements, such as
Columns, beams, slabs, etc.
B.2. Procedure
1.) Evaluation of necessary members to be scanned
- Columns and beams with the same tributary area, loadings and spans typically have the
same schedule; analysis of the structural will be performed to determine the highest
probability of members with similar schedules, to assure that efficient rebar scanning is
performed on critical sections
2.) Dismantlement of obstructing architectural claddings
- In the event that critical structural members are covered by architectural cladding (such
as ceiling covers), the concrete member will be stripped down of all obstructions until
the necessary dimensions are noted.
- Columns would be scanned for both vertical and transverse reinforcements at the mid-
section
- Beams and Girders would be scanned for both longitudinal rebars and stirrups at three
locations; at both supports and at the mid-span.
C.2. Procedure
1.) Locate Test Locations
- The total quantity of rebound hammer locations are determined in proportion with the
total concrete floor area of a building
- A total of 10 hammer blows per test location, a minimum of 1-inch separation from
each blow is required
- Test results are stored within the equipment memory, locations are recorded on the
As-built framing plan, for further analysis in the succeeding investigation process
D.2. Procedure
1.) Location Set up
- Locations are determined through the use of a rebar scanning, as to avoid hitting
rebars/steel reinforcements
3.) Restoration
- Sample holes are then restored using a higher grade concrete mix (epoxy-based
cementitious grout) by third party contractor under our team.
