is our family friend and townmate;

2. That we know for a fact that ESTELITA RIBAO DELA PEǸA is the
daughter of Spouses Felix Mata Dela Peña and Leonora Galit Ribao;

3. That we know for a fact also that ESTELITA RIBAO DELA PEǸA was
born on March 16, 1972 in Cabanatuan City, Nueva Ecija to the above
mentioned spouses;

4. That as per certificate from the Philippine Statistics Authority (PSA),

they found out that ESTELITA RIBAO DELA PEǸA has no record of
birth as per attached Negative Certification of Birth;

5. That we have executed this affidavit to set the record straight about
the facts indicated above and support the late registration of
ESTELITA RIBAO DELA PEǸA with the Cabanatuan City, Nueva Ecija,
Local Civil Registrar’s Office and for other legal purposes it may serve.

friend and townmate;
7. That we know for a fact that CARLITO ROSTROLLO SALVACION is the son of Spouses
Marcelo Salvacion and Gloria Rostrollo;
8. That we know for a fact also that CARLITO ROSTROLLO SALVACION was born on April
13, 1953 in Calauag, Quezon to the above mentioned spouses as per his Voter’s ID and
Senior Citizen ID;

9. That his middle name and YEAR of birth was ERRONEOUSLY recorded in the Social
Security System (SSS) as CRUZ (C) instead of ROSTROLLO (R) because he
unintentionally provided incorrect middle name and 1955 instead of 1953 respectively;

10.That as per certificate from the National Statistics Office, they found out that CARLITO
ROSTROLLO SALVACION has no record of birth as per Negative Certification of Birth;

11.That CARLITO ROSTROLLO SALVACION with date of birth on April 13, 1953 in his
Voter’s ID and Senior Citizen ID and CARLITO C. SALVACION with date of birth on April
13, 1955 in the SociaL Security System (SSS) records is one and the same person.
12.That we have executed this affidavit to set the record straight about the facts indicated
above and to establish that his true and correct name is CARLITO ROSTROLLO
SALVACION and the correct date of birth is April 13, 1953 and for other legal purposes it
may serve.

and townmates;
14.That we know for a fact that VICTORIA M. CORONEL is the daughter of Spouses
Avelino Coronel and Carmelita Mallari;
15.That we know for a fact also that VICTORIA M. CORONEL was born on December 20,
1936 in Cabanatuan City, Nueva Ecija to the above mentioned spouses as per her
Senior Citizen ID;

16.That as per certificate from the National Statistics Office, they found out that VICTORIA
M. CORONEL has no record of birth as per Negative Certification of Birth;
17.That we have executed this affidavit to set the record straight about the facts indicated
above and support the application of VICTORIA M. CORONEL for a passport and for
other legal purposes it may serve.
