RESERVATION AGREEMENT FORM

TERMS AND CONDITIONS

1. That Atom Ungson shall serve as and perform all duties and responsibilities of the master of ceremonies for
the Antonio-Caramoan which will be held at Avenue Plaza Hotel, Naga City on October 26, 2019.

2. Payment Details:
Total Professional Fee: Php 45,000.00
Reservation Fee: Php 45,000.00 -- Paid on: ______________ Received by: _________________

A. Payment of the reservation fee by the clients, along with corresponding signatures on this
document shall set this agreement in motion; a lack of any or all shall make this agreement
inactive and invalid
B. The reservation fee is nontransferable
C. The Total Professional Fee is INCLUSIVE of Atom Ungson’s transportation to and from
the venue of the event, his accommodation on the day of the event (if necessary), and out
of town additional charges.

3. Atom Ungson shall meet with the clients, preferably with the clients’ coordinator or event organizer, at a
date and venue that both Atom Ungson and the clients will agree to and set, between the time this agreement
is set in motion and the event itself, to discuss matters related to the event, i.e. the sequence and content of the
program, call time, attire, etc.

4. In case of cancellation of this agreement by the clients, made more than 60 days before the date of the event
the reservation fee shall be forfeited, as this is considered a loss in revenue. However, as a gesture of good
faith, should Atom Ungson be able to enter into a new agreement with another client on the same date of the
event on this agreement, the reservation fee, minus all nonrefundable costs already incurred, shall be returned
to the client who had sent the reservation fee.

In case of cancellation of this agreement by the clients, made 60 days or less before the date of the event, the
reservation fee shall be forfeited, and the balance must be paid up before the date of the event as this is
considered a loss in revenue. However, as a gesture of good faith, should Atom Ungson be able to enter into a
new agreement with another client on the same date of this agreement, all payments made, minus all
nonrefundable costs already incurred, shall be returned to the client who had sent the said payments.

5. In case of cancellation of this agreement by Atom Ungson, made more than 60 days before the event, the
client can choose to receive a full refund of payments already made or to be provided a replacement emcee,
subject to their approval, where the reservation fee shall then be used as reservation fee for the replacement
emcee and the client shall pay the balance of the replacement emcee.

In case of cancellation of this agreement by Atom Ungson, made 60 days or less before the date of the event,
the client shall receive a full refund of payments already made, and Atom Ungson shall provide a replacement
emcee at no cost to the client.

6. In the case where the clients decide to change the date of the event more than 60 days before the date of the
event, a new agreement shall be made, subject to Atom Ungson’s availability on the new date of the event,
and reservation fee shall be automatically transferred on to the new agreement. Should Atom Ungson already
be in an agreement with another client on the new date of the event, his original agreement shall take
precedence, and the client’s reservation fee is forfeited, as this is considered a loss in revenue. However, as a
gesture of good faith, should Atom Ungson be able to enter into a new agreement with another client on the
same date of this agreement, the reservation fee, minus all nonrefundable costs already incurred, shall be
returned to the client who had sent the reservation fee.

In the case where the clients decide to change the date of the event 60 days or less before the date of the
event, a new agreement shall be made, subject to Atom Ungson’s availability on the new date of the event,
and the original reservation fee is forfeited, while a new reservation fee is required to set the new agreement
in motion. Should Atom Ungson already be in an agreement with another client on the new date of the event,
his original agreement shall take precedence, and the client’s reservation fee is forfeited, and the balance must
be paid up before the date of the event, as this is considered a loss in revenue. However, as a gesture of good
faith, should Atom Ungson be able to enter into a new agreement with another client on the same date of this
agreement, the original reservation fee of the original date shall be considered the balance of the new
agreement on the new date, but any nonrefundable costs already incurred, that need to be rescheduled or
rebooked, need to be shouldered by the clients.

7. It is understood that the clients understand the importance of a reliable, professional, and experienced
sound system company with regard to the success of the event. The clients shall therefore, secure the services
of such, and with equipment inclusions that are sufficient and appropriate in consideration of the size and type
of the venue, and the floor layout of the event.

The clients shall also provide monitor speakers at the program area of the event, coursed through the sound
system company hired, and shall shoulder any fees, if necessary.

8. Atom Ungson will also announce the list of people to call during the pictorials after the ceremony. This
shall be further discussed in meetings previously set with the clients and Atom Ungson.
