hereby depose and state:

1. That our family residence is situated at the above mentioned address and on a parcel of land
duly registered under the name/s of ________________________________, as evidenced by

2. That our above mentioned family residence is deemed constituted as our Family home under
Article 152 and 153 of the Family Code.
3. That our family home is being mortgaged to secure a loan/credit accommodation in the amount

Address at ___________________________, including the renewals or extensions thereof or
additions thereto;
4. That as beneficiaries of the family home, we are aware of our rights under The Family Code and

that as provided in the Real Estate Mortgage, our family home maybe judicially and extra-

 AFFIDAVIT OF MARITAL CONSENT

accordance with law, do hereby depose and state:
