I am also providing you the Integrated Program and the Estimated Costs for the above
mentioned subject for your reference.

Procedures:

Meeting these objectives will require an open line of communication with me being the Project
Coordinator of AMA CONSOLIDATED and with the Project Coordinator. The success of this
project will be enhanced if you could identify a point person within your organization to provide
us with direction and answers to questions as needed.

 We will supply you with the contractors name and details


 We will be scheduling the Plan Preventive Maintenance (PPM) one (1) week prior to the
scheduled date.
 We will be presenting the complete Monthly Service Report.

Integrated Pest Management

We use Integrated Pest Management (IPM) principles as a tool in our pest control management program.
IPM is a very useful tool in reducing and controlling your pest problems. It involves educating clients, setup
of preventive measures to reduce pest, implementation of proper sanitation which includes removal of
pest harborage food and water, and lastly Chemical Treatment. The end goal of IPM is to reduce the
dependence in chemicals on pest control treatment.

General Pest Control service refers to control of common Crawling and Flying insect such as Cockroach,
Ants, Flies, Mosquito and Flea.

Rodent Control service refers to control of Mouse and Rat. We offer trap and poison treatment control

Termite Pest Control service refers to control of the most destructive insect to our
property. We offer both Pre-Construction and Post Construction Management for termite. PEST
CONTROL

AMACON will control Public Health Insect and Rodent pests to all common areas of
DATACOM through providing highest industry standard Integrated Pest Management methods.
AMACON will provide trained Technicians and technical support, all application equipment, pesticide
and standard consumable material. Client will support overall control efforts by ensuring the property is
properly maintained in order to deny pest attraction, entry, and proliferation, through good building
hygiene and maintenance practices.

ON-GOING ROUTINE PROGRAM

In order to maintain control of pests AMACON will on-going Monthly inspections and
treatment as needed during the contract period.

Once a Month: Insects and Rodents Public Health Pests – Inspection and treatment to
all common areas of DATACOM servicing at 60 Rodent bait stations.

EMERGENCY SERVICES AS REQUIRED

AMACON will provide emergency call response service and spot treatment and as when
needed. This will be limited to 3 call outs during this contract period.

PESTS COVERED

Within the Pest Premiere Plan service AMACON will control rats and mice, ants, bed
bugs, flies and mosquito breeding sources, cockroaches, ticks, flees, harmful spiders, scorpions,
bees/wasp and hornet nests.
Separate charges will apply for on-going treatment against flying insects coming from
outside the property, wild cats and dogs, snakes, termites and other wood boring insects, birds, bats and
garden plants feeding pests.

DOCUMENTATION

Within our service we supply AMACON Pest Control Record folder that will be kept on
the site with the Clients’ representative who over-look the Pest Control Contract. This information is
designed to be informative of the work and allows easy reference to the client, authorized auditors and
the municipality.

COMPREHENSIVE PEST CONTROL MONTHLY CHARGE

PhP 12,000/ Per month

REGULAR PEST CONTROL MONTHLY CHARGE

PhP 9,500/ Per month

2. The Services will also include any other tasks which the Parties may agree on. The
Service Provider hereby agrees to provide such Services to the Customer.

Term of Agreement

3. The term of this Agreement (the “Term”) will begin on the date of this Agreement
(date) ________________________ and will remain in full force and effect indefinitely until
terminated as provided in this Agreement.

4. In the event that the “Client” wishes to terminate this Agreement, that Party will be
required to provide 30 day notice to the other Party. ”Service Provider”

5. Except as otherwise provided in this Agreement, the obligations of the Service


Provider will end upon the termination of this Agreement.

Performance

6. The Parties agree to do everything necessary to ensure that the terms of this
Agreement take effect.
Currency

7. Except as otherwise provided in this Agreement, all monetary amounts referred to in


this Agreement are in PhP (Philippine Peso).

Compensation

8. For the services rendered by the Service Provider as required by this Agreement, the
Client will provide compensation (the “Compensation”) to the Service Provider as follows:

• The Customer will pay the Service Provider per Terms of Payment (divided into
4 collections) agreed. Each project has its own costs and the Service Provider agrees to inform
the Customer what are the costs involved when setting the quotation and the Customer agrees
to pay the total amount when the project is delivered.

Total Annual Value PhP 144,000/ (Comprehensive)

Value Per Collection PhP 114.000/ (Regular)

9. The Compensation will be payable upon completion of the Services, should there be
additional services occur outside this agreement.

10. The Service Provider is responsible for paying any Superannuation Guarantee
contributions that may be required in relation to the work performed by the Service Provider or
by employees of the Service Provider under this Agreement.

11. The above Compensation includes all applicable sales tax/VAT, and duties as
required by law.

Provision of Extras

12. The Customer will not provide any resources, assistance or extras for use by the
Service Provider in providing the Services.

Reimbursement of Expenses

13. The Service Provider will not be reimbursed for expenses incurred by the Service
Provider in connection with providing the Services of this Agreement.

Confidentiality
14. Confidential information (the “Confidential Information”) refers to any data or
information relating to the business of the Customer which would reasonably be considered to
be proprietary to the Customer including, but not limited to, accounting records, business
processes, and client records and that is not generally known in the industry of the Customer
and where the release of that Confidential Information could reasonably be expected to cause
harm to the Customer.

15. The Service Provider agrees that they will not disclose, divulge, reveal, report or use,
for any purpose, any Confidential Information which the Service Provider has obtained, except
as authorized by the Customer. This obligation will survive indefinitely upon termination of this
Agreement.

16. All written and oral information and material disclosed or provided by the Customer
to the Service Provider under this Agreement is Confidential Information regardless of whether
it was provided before or after the date of this Agreement or how it was provided to the
Service Provider.

17. AMACON will be providing a 5% discount for the signed Annual Maintenance
Contract (AMC)
