AFFIDAVIT OF CANCELLATION OF ENTRY

I, RAQUEL CENTINA y MARIANO, of legal age, Filipino, married, and a

resident of Barangay Camanci, Batan, Aklan, Philippines, after having been duly
sworn to in accordance with law, hereby depose and state, that:

1. That LEOPOLDO DE MARIANO had seven (7) children, namely: Elisio Mariano,
Ploro F. Mariano, Lauro F. Mariano, Raquel Centina y Mariano, Nancy Parreno y
Mariano, Lera Dalida y Mariano, and VIola Lebadesos y Mariano;

2. That I am one of the lawful heirs of the true and registered owner of a certain
parcel of land which is covered by Original Certificate Title No. P-19488, more
particularly described as follows:

LOT NO. 3022, Psc-35

Containing an area of FIVE THOUSAND FOUR HUNDRED AND

NINE (5,409) SQUARE METERS. Bounded on the NE., along line
1-2 by Lot 3021, Psc-351 on the E., along line 2-3 by Lot 3080,
Psc-35; on the SE., along line 3-4 by Provincial Road; and on the
SW., and W, along lines 4-5-1 by Lot 2646, Psc-35.

3. That annotated on the said Original Certificate Title No. P-19488 are the
following entries, to wit:

AFFIDAVIT OF LOSS: EXECUTED BY EVA G. FRANCISCO, ATTESTING THAT
THE OWNER’S DUPLICATE OF TITLE P-19488 WAS LOST IN ACCORDANCE
WITH DOC. NO. 228, PAGE NO. 47, BOOK NO. 18, SERIES OF 2018, OF
NOTARY PUBLIC OF KALIBO, AKLAN, ATTY. JERALD I. ILEDAN, DATED
FEBRUARY 22, 2018.

AFFIDAVIT OF LOSS: EXECUTED BY EVA G. FRANCISCO, ATTESTING THAT
THE OWNER’S DUPLICATE OF TITLE P-19488 WAS LOST IN ACCORDANCE
WITH DOC. NO. 386, PAGE NO.79, BOOK NO. 25, SERIES OF 2018, OF NOTARY
PUBLIC OF KALIBO, AKLAN, ATTY. JERALD I. ILEDAN, DATED APRIL 12, 2018.

4. That the entry aforementioned falsely asserts that the Owner’s Duplicate of Title
P-19488 is lost because there exists no Owner’s Duplicate of Title P-19488;

5. That I am in fact in possession of the Original Certificate of Title P-19488 as one

of the lawful heirs of the original owner, Leopoldo de Mariano;

6. That I am executing this affidavit purposely requesting the Register of Deeds to

cause the cancellation of the above-mentioned entries in the aforementioned
Original Certificate of Title.
