INFORMATION

The undersigned, Assistant Provincial Prosecutor, upon sworn complaint

originally filed by the offended party, accuses RYAN A. LEOCARIO of the crime
of FALSIFICATION OF PUBLIC DOCUMENTS, defined and penalized under
Article 124, par. 1(b), of the Revised Penal Code, as amended, committed as
follows::

1. That sometime in the first week of June 2018, at the Polytechnic

University of the Philippines College of Law Library, the above-named
accused, did then and there, willfully, unlawfully and feloniously falsify
the document of the University Registrar.

2. That the accused entered in the office of the Registrar on July 02, 2018,
around 8 a.m. taking with him some documents like Transcript of
Records, et cetera.

3. That the accused changed all his grades to 1.0.

4. That the evidence shows that the altercation of the document is reflected
on the face of the said document after thorough investigation.

has been committed and that the accused is probably guilty thereof; that the
accused was informed of the complaint and of the evidence submitted
against him and was given an opportunity to submit controverting evidence.

A preliminary investigation has previously been conducted in this case under

my direction, having examined the witnesses in accordance with the
provisions of R.A. No. 5180, as amended by P.D. No. 77, Dec.6, 1972 and
P.D. 911, March 23, 1976 and as implemented by Dept. of Justice Circular
No. 74, series of 1967 and Circular No. 23, series of 1975.
