TOTAL CONTRACT PRICE (VAT -EX) 5,823,084.00

For and in consideration of the above services stated in item 1.0, our fee to include salaries,
personnel, equipment rental/depreciation and all other expenses incident to the accomplishment
of all the works required of us shall be lump sum FIVE MILLION EIGHT HUNDRED TWENTY
THREE THOUSAND EIGHTY FOUR PESOS ONLY (PHP 5,823,084.00) exclusive of vat and other
taxes.

2.0 PERFORMANCE PERIOD

1. We shall mobilize our field personnel within four (4) days upon receipt of your purchased
request/order for inspection and down payment.

2. We shall complete the work within thirty (90) working days reckoned from the time that
we have mobilized our equipment and manpower at the project site.

3.0 TERMS OF PAYMENT

1. 40% down payment

2. Rest is progress billing based on actual accomplishment
3. Down payment shall be deducted from each billing on prorated condition based on
actual accomplishment
4. 10% Retention shall be in effect however it shall be release after 30 days upon
acceptance of the works

4.0 VALIDITY OF PROPOSAL

1. Ninety (90) days

5.0 OTHER CONDITIONS

1. The contractor shall provide for all equipments, materials, labor, tools and other
equipment necessary to complete the above-mentioned project described herein, to
the satisfaction of DEGIC
2. The workmanship must be of high standard of quality and must be made by
specialist and experts in all aspect of the work

 3. The contractor shall guarantee high level workmanship and high level reporting.
4. Existing utilities, materials or properties if damaged due to negligence or fault of the
Contractor during and after the project completion shall be repaired or replaced by
the Contractor at his own expense.
5. Safekeeping of all service materials, tools, instruments and equipment shall be the
responsibility of the Contractor and any loss of or damage shall be at his own
expense.
6. The Contractor shall be responsible for the actions of his employees at all times and
shall be liable for any damage they may cause or misconduct that they make while
inside Sanyo Plastic compound. Any injury to any of the contractor’s persons shall
be the sole responsibility of the contractor and not of DEGIC and the contractor shall
assign the necessary handling of injuries to his person.
7. Any ambiguity, discrepancy or query shall be referred to DEGIC for clarification and
decision.
8. The work shall include cleaning of project site of debris and excess materials as the
work progresses and after the completion of the work. The work area must be kept
clean and orderly at all times.
9. Contractor shall be responsible for the board and lodging of his personnel outside
the plant.
10. The contractor shall provide his employees with PPE and to make sure it will be
used at all times while in the work area.
11. The Contractor is obliged to strictly observe safety measure in all aspect of work.
12. Contractor’s personnel will be required to submit clearance that may be deemed
required by the Institute, Security Office.
13. NO ID, NO ENTRY policy will be imposed to all contractors’ personnel
14. Electricity and water will be provided by client free of charge
