90 Day Evaluation

Over the next 90 days you will be evaluated on the following and be
evaluated according to the following tier :

After 30 days of training and in-office experience, employee should be

able to successfully complete following tasks by themselves:

 Rooming and greeting patients

 Obtain patient history and report to physician
 Remove dressing (if needed) and re-apply at the end of visit
 Able to successfully complete at least majority of note for physician
 Completely set up for a procedure and assist as well
 Clean after themselves, restock exam rooms, autoclave surgical
instruments
 Faxing clinical documents, calling pharmacies
