Who may avail of the service:


Any person may file an administrative complaint against any DepEd
official or employee at the Office of the Schools Division
Superintendent.

What are the requirements:


Number of copies: Minimum of four (4) sets
The complaint and its annexes shall be in such number of copies as
there are respondents, plus two (2) copies for the official file.

Form of Complaint
Under oath and written in a clear, simple and concise language so as
to inform the person complained of, about the nature and cause of
the accusation against him.

Content of Complaint
1. Full name and address of the complainant;
2. Full name and address of the person complained of, as well as
his position and office in the Department of Education;
3. A narration of the relevant and material facts which show the
acts or omissions as allegedly committed by the person;
4. Certified true copies of documentary evidence and affidavits of
his/her witnesses, if any; and
5. Certificate of Non-Forum Shopping.

Content:
The MR shall state the grounds relied upon.

Limitation:
Only one (1) MR shall be allowed.

requirements stated in DO 49, s. 2006.

Disciplining Authority (DA)

Investigator/s commence the FFI or PI by ISSUING an ORDER requiring


the person complained of to submit WITHIN THREE (3) DAYS from receipt
of order a COUNTER-AFFIDAVIT/COMMENT under OATH and affidavits of
witnesses of the person complained of with supporting documents.

CONFERENCE
1. To propound clarificatory questions to the parties; and
2. Interview the witnesses of the parties.
