JOINT AFFIDAVIT

WE, GAUDENCIO C. CABAJON, of legal age, married, Filipino and with residence and
postal address at, Bgy. Cangmating, Sibulan, Negros Oriental, Philippines, and CONCEPCION
D. DIPUTADO, of legal age, married, Filipino and with residence and postal address at Bgy.
Magatas, Sibulan, Negros Oriental, after having been sworn to in accordance with law, do
hereby depose and say:

That we both personally know the person of CARLING DURAN CALISAÑGA as we are
close associates and friends of the family;

That only recently, we were shown a copy of the MEMBER DATA RECORD OF SOCIAL
SECURITY SYSTEM of CARLING DURAN CALISAÑGA wherein his family name was
erroneously entered as “CALISANGA” instead of “CALISAÑGA”, likewise there is no entry of
his middle name “DURAN” because it was inadvertently omitted per registration with SSS;

That, we can both unequivocally state and manifest that “CARLING DURAN
CALISANGA” as appearing in the Member Data Record of Social Security System and
“CARLING DURAN CALISAÑGA” as appearing in the Certificate of Live Birth and Baptismal
refer to one and the same person; whose true and correct name to be retained on file is
“CARLING DURAN CALISAÑGA”;

That we both know these facts to be true and correct because we are close associates
and friends of the said “CARLING DURAN CALISAÑGA”, and his family;

That we are executing this affidavit without mental reservation to attest to the veracity
of the above-stated facts and for whatever legal purpose this may serve best.

IN WITNESS WHEREOF, we hereunto affix our signatures this 13th day of November,
2014 at Sibulan, Negros Oriental, Philippines.
