PETITION
PETITIONER, through counsel, unto this Honorable Court, respectfully states that:
 1. Petitioners, KARA SY and JAKE SY, Filipinos, both of legal age and
residents of 23 Aznar Road, Sambag 1, Cebu City where they may be served
notices and other processes of this Honorable Court.

2. Respondent, Philippine Statistics Authority/ Office of the Civil Registrar

General is a government agency, with office address at 3rd Floor, Vibal Building,
Times Street corner EDSA, West Triangle, Quezon City, where it can be served
with the orders and processes of this Honorable Court.

2. Petitioners are legally married to each other. During their union,

petitioners begot a child, who they named CEDRIC SY. Immediately after his
birth, petitioners reported and registered CEDRIC SY’s fact of birth with the
Office of the Civil Registrar.

3. However, by sheer oversight and inadvertence, petitioners mistakenly

entered the year 2012 in the Affidavit instead of 2011. On the basis of petitioner’s
Affidavit which bears an erroneous entry anent Cedric Sy’s year of birth, the
Certificate of Live Birth, thus, reflects the year 2012, as Cedric Sy’s birth year.

WHEREFORE, it is respectfully prayed that an order be used directing the

Philippine Statistics Authority/Office of the Civil and Registrar General, to rectify
the entry in the birth records of Cedric Sy anent his birth year, so as to change it
from 2012 to 2011.

All other just and equitable reliefs are also prayed for.

3 We have read and understood the contents of the foregoing petition which
are true and correct and based on our personal knowledge and on some
authentic records.
4 We hereby certify that I have not heretofore commenced any petition nor
filed any action involving the same issues, subject matter, and parties before
the Supreme Court, the Court of Appeals, or different divisions thereof, or
any court, tribunal or quasi-judicial agency, and to the best of our
knowledge, no such other action, petition or claim is pending therein; If we
should hereafter learn of such other similar action, petition, or claim, then
 we shall report such fact to this Honorable Court, within five (5) days from
knowledge thereof.
