SUBJECT: Implosion of 630 Alton Road site ( South Shore Hospital)

There have been some questions regarding the implosion permit for the South Shore
Hospital site. The purpose of the LTC is to provide the most complete up to date
information.

Assistant City Manager Eric Carpenter coordinated all the City departments Police, Fire,
Public Works, Building and Tourism & Culture to identify the least impactful date and time for
the implosion, coordinating around traffic, special events and safety concerns.

The Building Department has been in contact with the contractor BG Group LLC
Steven Greenberg) throughout the process. Blaster Controlled Demolitions Inc.,
Field operations manager Thom Doud) will be on site throughout the preparation

and blasting process.

The Public Works Department has reviewed the necessary road closures and MOT
as well as the facilities protection report and the vibration monitoring plans provided
by the engineers. Both the implosion demolition plans, and the necessary road
closure permits have been approved.

The Fire Department Fire Prevention Division has reviewed the storage magazines
and the distances to buildings and roadways.

Miami- Dade County Environmental Resource Management has reviewed the plans,
including the asbestos survey pursuant to 40 CFR 61 subpart M ( National Emission
Standards for Hazardous Air Pollutants) and F. S. 469. 001- 015, of the existing
building and has approved the permit for demolition by implosion.

The Police Department will secure all the necessary exclusion zones to ensure the
safety of residents in the surrounding buildings and area. The entire process should
take between one — two hours. The sequence of events is attached.

Both the contractor and the owners have been providing notices ( Attachment # 1) to the

neighboring properties. A pre- implosion meeting was held last Thursday April at
6: 00 p. m. ( Attachment # 2). The contractor has also provided a detailed list of the areas and
properties and buildings to be protected ( Attachment # 3). Steven Greenberg has indicated
that the dust should dissipate within minutes of the implosion. The Marketing and
Communications Department will be disseminating this information using our social media
platforms and traffic text system.

Finally, the structural modification permit BC1910377 had its approved final inspection prior
to issuance of the demolition permit. The County Blast Permit ( issued by Miami- Dade
Department of Transportation and Public Works Construction Division) is required by the city
prior to allowing the explosives to be brought to the site and can be issued now that the
building permit is issued.

Demolition, Inc. (" CDI" - Implosion Subcontractor) would like you to be aware that on April 16, 2019, the
vacant South Shore Hospital will be demolished by implosion.

Implosion is a process where a small amount of explosives is used to disrupt selected supports in a building.
This allows gravity to pull the structure down in a controlled manner. CDI has used this implosion method

thousands of times around the world during the past 65 years to assist Property Owners and Contractors in
the removal of unwanted structures.

There are three ( 3) points we would like to make you aware of:

1. Dust is an unpreventable byproduct of all types of demolition. With a wrecking ball or high- reach
excavator, the building is slowly broken up releasing dust over a lengthy period of time. With implosion,
the same overall quantity of dust is released in a matter of seconds, and may linger in the general area
for 4 to 6 minutes before crews begin cleanup of same from streets and sidewalks. Depending
on wind speed and direction on the day of the implosion, the dust may or may not reach your properties.
As a precautionary measure, we recommend you see that all windows, doors and air intakes are closed,
and cover any other openings that might allow dust to enter your apartment, house or building.
We also recommend you turn off all exhaust fans before or at the time of the implosion as these
might draw air in to your building through cracks in walls and windows.

2. Early Tuesday ( April 16th) morning, the City of Miami Beach Police Department will set up an
Exclusion Zone around the buildings. This is to prevent people from getting too close. We need your
cooperation with the Police and contractors to ensure this is a safe job. Everything they say to you or
ask of you is to minimize inconvenience or to promote safety.

3. There will be heavy dust in the block immediately surrounding the demolition site. Dust may also drift
outside and downwind of the Exclusion Zone, depending on weather conditions. If you find dust

uncomfortable or irritating, or if you have any respiratory conditions that would be aggravated by dust,
please stay indoors during the demolition.

4. More than 100 years of vibration/ noise monitoring data assembled by the US Bureau of Mines
established guidelines for safe vibration and noise levels around blasting sites.
For several days prior to the implosion, a pre- demolition photographic survey will be made to document
the pre- existing condition of adjacent structures and properties in accordance with State Law. Noise and
vibration monitoring instruments will be set up around the demolition site to monitor actual levels of
same generated by the implosion/ fall of the South Shore Hospital structure.
A follow- up post- implosion survey will be made the morning of the implosion to determine whether or
not any changes have taken place in the structures previously inspected, as a result of the implosion.
The vibration and pre/ post- survey reports go to the State of Florida to protect the interests of all
parties involved.

Dear Ana:

I walked the surrounding neighborhood Tuesday and noted the following items to be addressed:
CID Muzzafiato Building. There are 6 doors to be protected that we, The BG Group ( BG), will seal

with 3" blue painters tape*.

i The Bentley, 520 Alton. The front glass doors on the north side will be closed and taped. *
The Bentley, 540 Alton. A BG employee will remain in the lobby to remove tape after blast.
o South side. The front glass doors will be closed and taped. *
o North side. There are 3 doors to be taped and 1 vented door that will be sealed with
poly sheeting and tape.*
The Floridian. BG will tape and have an employee remain in lobby to unseal the following.*
o South side. 2 doors and 3 vents to be protected.
o East side. 1 door and 5 vents.
Oo Main entrance. 1 door
IME o Northeast side. Vented doors will be completely sealed.
Receiving area. 2 doors and a window AC unit to be wrapped.

Community Health Center. We will wrap roof AC units if permitted.


Baptist Health. BG will tape and unseal 1 door on the west side.*
O CubeSmart. Spoke with owner Ken Edelman. He will not open until after the blast. BG will
tape and unseal the following.*
o West side. 2 doors
o South side. 2 doors
o Roof. Wrap AC units
St. Francis Church. AC units and fan/ duct will be wrapped and unsealed by BG.* Spoke with

office manager who will speak with maintenance about turning AC off and if any additional
protection is desired. The doors are all wood and very secure.
Publix. Met with manager and asst. manager. Doors will be locked—9: 30 and customers will
be allowed to leave via the emergency doors on the east side. We will have a BG employee in
the store during the blast to unseal and remove the tape after the dust clears.*
o North side. 4 '/ 2 doors and one ATM to be protected
o West side. 1 door. 2 100 yd. trailers will be parked under the Alton Rd. overpass.
