‘SAFE WORK METHOD GUIDELINES - ENVIRONMENT, HEALTH AND SAFETY (EH&S) 1, Mechanical & Electrical Services 1.1 Contractors shall not alter, shift or tamper in any way with the existing structure or system or any ‘Mechanical & Electrical (M&E) service (whether concealed or exposed) within the Site or common, area without prior written approval of Mitsui Outlet Park KLIA Sepang (MOP), Jabatan Bomba and Penyelamat Malaysia or any other relevant authorities; 1.2 Any temporary electricity or water sources will be provided at specific locations upon request and necessary payment to MOP; 1.3. Tapping off the electricity or water supply without MOP prior permission is prohibited and MOP has the discretion to suspend the work order; 2. Cleanliness 2.1 Contractors must ensure that the site is clean at all times during the execution of the Works; 2.2 Allrubbish and debris arising out of the works are to be removed and disposed properly and safety off-site; 23. Loading and unloading must be made in such a manner so as not to leave any stain nor create a mess In any part of the common area on a daily basis; 24 The Contractors shall ensure that its workers or subcontractors uses designated tollet located at Backend, Ground Floor and do not abuse the toilets feclities and shall keep the toilet clean and proper manner. Anyone found urinating, defecting or spitting in any part of MOP building will be removed from the working area without exception immediately; 2.5 Washing and cleaning themselves and tools or any equipment in the toilet are strictly prohibited; & 2.6 Smoking is strictly prohibited at any part of the Site; 3. Loading and Unloading 3.1. Allloading and unloading of materials, equipment and /or rubbish /debris MUST be carried out at the designated areas. Contractors are required to ensure that delivery carts and barrows are fitted with rubber rollers so as to not damage the flooring and overloading to the delivery carts are prohibited; 3.2. Materials must NOT be dragged so as to cause any marking or damage to the flooring: 3.3 Floor protective must be provided for the full distance between the loading and delivery locations; & 3.4 Any delivery of heavy, bulky, fragile or sharp edge item such as tank, container, constrictions equipment or machinery shall only be made after prior written approval of Mitsui Outlet Park KLIA Sepang had been abtained; and 4. Standard PPE Requirements 4.1 Environment, Health & Safety sets out the obligation of any workers where it is not practicable to ‘avoid the presence of hazards atthe site or workplace; 4.2 The Supervisor in-charged is /are responsible to ensure all risk and protection system are appropriate:- 42.1 Foot Protection - working with sharp objects, expose to molten metal, working on or around hot, wet or slippery surface and on electrical hazard presents ares; 422 Safety Helmets - all workers are required to wear head protection to prevent object falling from above and possible accidental head contact with electrical hazards; 42.3 Hand Protection (Glove) - protection against the identified risk such as type of chemical handled, nature of contact (total immersion, splash, etc), thermal protection, abrasion and cut resistance; 424 Respiratory Equipment - protections against insufficient oxygen environments, harmful dusts, fogs, smokes, mist, gases, vapours and sprays; 425 Face & Eye protection - protection against dust, drt, metal or wood chips entering the eyes, chemical splashes from corrosive substances, hot liquids, solvents and radiant energy from welding, harmful rays from lasers or other radiant lights; & 426 Visible Vest / Jacket - essential protective for greater visibility under inclement situation where workers perform works close to trafic, heavy equiament or in weather obscures “Ihave read and agreed to adhere to the guidelines stated above, Applicant Signature Page 3 0f 3
