Instructions for use of kit 1. Determine if the patient would do better with higher or
lower level questions.
- Ex. of higher level question: “You have a Dr. apt
tomorrow at 10:30am. It takes 30 minutes to get
from your house to the Dr.’s office. What time do
you need to leave your house? Set the clock for that
time.”
- Ex. of lower level question: “ Find Cincinnati on the
map.” OR “What time is it?”
2. If higher level questions are appropriate, use the cards
outlined in BLUE. If lower level questions are
appropriate, use the cards outlined in ORANGE.
3. Either give the card to the patient to read and follow,
OR read the card to the patient.
4. The materials needed for each activity are listed on the
back on the question cards
5. Prompt and assist the patient as necessary throughout
completion of activity.
Examples of intervention 1. Set the clock to 3:30
activities 2. What time do you usually eat dinner? Set the clock for
that time.
3. Find Cincinnati (On Ohio Road Map)
4. You have a doctors apt tomorrow at 10:30am. It takes
30 min to get from your house to the Dr.’s office. What
time do you need to leave your house? Set the clock for
that time.
5. What states would you drive through to get from Ohio
to Colorado?
How kit can be graded for low High Level Patients (Use BLUE cards)
and high level patients - Let patients find what materials they need to
complete the task (written on back of card).
- Give the cards to the patient to read and complete
on their own.
- Question Examples:
o Which highway would you take to get from
Columbus to Cincinnati?
o You have a doctors apt tomorrow at
10:30am. It takes 30 min to get from your
house to the Dr.’s office. What time do you
need to leave your house? Set the clock for
that time.
o What states would you drive through to get
from Ohio to Colorado?
o Write the following events on the weekly
calendar. Follow up appts: Monday 8am @
Ross Heart Hospital, Wednesday 3:30pm @
Ohio Stadium. Family Events: Billy’s
baseball game on Tuesday 6pm, Family
dinner on Sunday at 7pm
Low Level Patients (Use ORANGE cards)
- Set the materials needed for activity in front of
patient.
- Read the card to the patient before giving it to them.
- Question Examples:
o Find Cincinnati
o What time do you usually eat dinner? Set the
clock for that time.
o Set the clock to 3:30.
o Find Franklin County.
o What time is it? OR What time does the clock
read
