AFFIDAVIT OF GUARDIANSHIP

THAT I, CLEOFE SALIN SAYCON, of legal age, Filipino, widow, and a

resident of Enrique Villanueva, Sibulan, Negros Oriental, after having been duly sworn to
in accordance with law, do hereby depose and manifest:

That I am the mother of CLEABENECA SALIN SAYCON, 14 years old, and

LEAH BIANCA SALIN SAYCON, 10 years old, who are both minors;

That the father of CLEABENECA SALIN SAYCON and LEAH BIANCA

SALIN SAYCON died last December 21, 2018 at Bagtik, Enrique Villanueva;

That I stand as the sole guardian of CLEABENECA SALIN SAYCON and LEAH
BIANCA SALIN SAYCON since their father died and until the present, and I assumed
all the responsibilies of being their legal guardian and supported all their basic necessities
in life, since birth and until the present;

That this affidavit is executed freely and voluntarily and to attest the veracity and
truthfulness of the aforementioned statements herein.
