JOINT AFFIDAVIT

That we, ______________ and_________________, both of legal age, Filipinos,

married and residents of barangay Magatas, Sibulan, Negros Oriental, under oath, do
hereby depose and manifest:

That we personally know ALEJANDRO DONGCOY CABALIDA, who was born

on May 3, 1934, as shown in his Baptismal Certificate;

That in his Social Security System’s Record, his Middle Name was erroneously
entered as Ducoy, instead of Dungcoy as shown in his Baptismal Certificate;

That the name ALEJANDRO DONGCOY CABALIDA appearing in his

Baptismal Certificate and the name ALEJANDRO DUCOY CABALIDA appearing in his
SSS Records, refers to just ONE and the same person;

That we know all this facts because we are good friends and neighbors in
Barangay Magatas, Negros Oriental for quite sometimes until he died ; and we executed
this affidavit freely and voluntarily without any considerations or promise whatsoever nor
to defraud any office of the government but to attest the truthfulness and veracity of our
statements herein.

IN WITNESS WHEREOF, we have hereunto affixed our signatures this 3 rd day of June,
2014, in Dumaguete City, Negros Oriental, Philippines.
