Full Incidents Investigation Report

The incident was reported to be occurred last Wednesday, and as it is reported a client who
apparently was is in hurry used emergency stairwell instead of using lift where she fell down and
broke her wrist. As it is, report is that the stairs were blocked by boxes of paper. It is also mentioned
that one of the agents heard the call for help and as he opened the door to help without realizing the
situation he actually complained about her wearing high heels and implied that such thing would not
have happened to any sensible woman or man. As for medical help the client was taken to royal
woman’s hospital by ambulance. The client did contacted back with possible to sue MLR for injuries,
loss of income and mental trauma.

As in further investigation, it is also seen that another agent did reported a strange odor and an oily
substance on the stairwell about which he notified the commercial reality manager. HR manager who
is contacted by client’s lawyer and office of human rights ombudsman was not aware of incident
earlier and was just made known to this. Investigation has found manager’s failure to undertake WHS
management system responsibilities where recent incident reports are incomplete and no
consultation with agents and WHS committee has occurred.

A customer chose to leave the structure by means of the crisis exit and utilize the

the customer felt down the stairs and broke her wrist. The operator who heard her

Hospital by rescue vehicle.
