is registered on the name of “VINODBHAI R” on ESIC portal. However, sticking to the fact
and truth, Vindodbai Balwanthai Parmar’s contributions towards ESI are being paid from this
IP No. only, though the name submitted is inadequate by mistake. Thus, this is to clarify that,
Vinodbhai Parmar is sole and actual holder of IP No. and subscriber of ESI. He has made
