AFFIDAVIT OF DESTRUCTION/MUTILATION OF PASSPORT

I, MANJINDER SINGH, of legal age, single, Indian citizen, and with residence
address at Sitio Tambis, Pook, Talisay City, Cebu, Philippines, after having been duly
sworn to in accordance with law hereby depose and state that:

1. I am a holder of an Indian Passport with Passport No. M9563788 issued

on 13 May 2015 and valid until 12 May 2025 issued by the Republic of
India;

2. The said passport was damaged on or about the 19th day of March 2019
because I accidentally dropped it in the toilet due to excitement;

3. When I picked it up and tried to dry it, the same already became
unreadable;

4. The damage to the passport issued in my name was inadvertent and

unintentional;

5. The damage on the said passport has rendered it unusable for travel
purposes and therefore I request the Embassy of India in Manila,
Philippines to issue a replacement in my favor; and

6. As such, I am executing this Affidavit of Destruction/Mutilation of

Passport to attest to the truth of the foregoing statements and to request
the issuance of a new Indian Passport from the Embassy of India in
Manila, Philippines in lieu of the one which was damaged.
