JOINT-AFFIDAVIT

We, RICARDO P. DONAYRE and ROLANDO P. SULLANO, JR. both of legal

age, Filipino citizens, and residents of Brgy. Curva, Ormoc City, Leyte, Philippines, after
having been duly sworn to in accordance with law, hereby depose and say, THAT:

1. We personally know FAMILA T. PEÑON as she is one of our close associates

in Ormoc City, Leyte;

2. However, in the records with the Certificate of Live Birth of her children as
well as in their NSO records, her first name is erroneously entered as PAMELA when in
truth and in fact her first name is FAMILA as what she is known to have and as borne
out by her records in her Voter’s Certification and Certificate of Marriage;

3. We are executing this affidavit to attest to the veracity of the foregoing facts
and circumstances, to establish the fact that the true and real first name of Famila T.
Peñon is FAMILA; that the names PAMELA T. PIÑON as appearing in her Children’s
Certificate of Live Births and the name FAMILA T. PEÑON as appearing in her records
with the Commission on Election refer to one and the same person, our friend in Ormoc
City, Leyte and for whatever legal purpose this may serve.
