Contract of Services

KNOW ALL MEN BY THESE PRESENTS:

This contract made and entered by and between:

Barangay Inzo Arnaldo Village, through the office of the Punong barangay with office address at
barangay Inzo Arnaldo Village, Roxas City, herein represented by HON. EINSTEIN CHINUMEZI T.
OKPORA Punong barangay, herein referred to as the “First Party”;

-and-

______FRELUZ D. DELA CRUZ____, of legal age, Filipino and with residence address
at_______Brgy. Inzo Arnaldo Village, Roxas City, herein referred to as “Second Party”;

WITNESSETH

1. That the first party is in need of the services of the Second party who shall perform work not
performed by the regular personnel of the First Party;
2. That the Second party has signified his/her intention, to which the First party has accepted, to
provide the service needed by the latter;
3. That the Second Party hereby possesse the education, experience and skills required to perform
the job as described herein;
4. That the second Party hereby attests that she is not related within the fourth degree of
consanguinity or affinity to the: 1.) Hiring authority and/or 2.) representative of the First Party;
that she has not been previously dismissed from government service by reason of an
administrative offense; that she has not already reached the compulsory retirement age of sixty-
five (65);
5. That in view hereof, the Second party is hereby Contracted as a ___VAWC Desk Officer___ __
at _______Barangay Inzo Arnaldo Village _____ the period of August 01 until ___December
31, 2018____ at the rate of Three Thousand Pesos (3,00.00) to be paid on a monthly basis
provided that the actual work rendered should not less than 22 working days in a month (8 hours
is equivalent to one (1) working day.)
6. That as ____VAWC Desk Officer___, the Second party is expected to perform the following
functions:
1. Respond to gender-based violence cases brought to the barangay;
2. Record the number of gender-based violence handled by the barangay and submit a
quarterly report of all cases of VAWC to DILG Field Office and CSWDO;
3. Keep VAWC records confidential and secured , and ensure that only authorized
personnel can access it;
4. Assist victims of VAWC is securing Barangay Protection Order (BPO) and access
necessary services;
5. Develop the barangay’s gender-responsive plan in addressing gender violence,
including support services, capacity building and referral system;
6. Coordinate with and refer cases to government agencies, NGO’s, institutions, and
other services providers as necessary;
7. Address other forms of abuse committed against women and children, especially
senior citizens, women with disabilities and other marginalized groups;
 8. Lead advocacies on the elimination of VAWC in the community; and
Perform other related functions as may be assigned
7. That the Second party shall perform work at time and schedule to be agreed upon by both
parties;
8. That it is understood that this contract does not create an employer-employee relationship
between the First Party and Second Party, that the service rendered hereunder are not considered
and will not be accredited as government service; and that the later is not entitled to benefits
enjoyed by the regular personnel of the First Party;
9. That the compensation of the Second Party shall be taken from the funds of the Barangay

IN WITNESS WHEREOF, both parties have hereunto set their hands this August 1, 2018 at Roxas
City, Philippines.
