AFFIDAVIT OF CONSENT

We, FERDINAND C. SILLONA and MARGIE SILLONA of legal age, married, Filipino and resident
of No. 7 Laredo St .Concepcion Dos , Marikina City after having been duly sworn to in
accordance with law do hereby depose and state;

1. That we are the biological parents of the minor ROSLIN SILLONA, born on June 16, 2006;

2. That the child will travel to France, Italy, Austria ,Czech Republic and Slovakia for
tourism and leisure trip;

3. That the mentioned child will be leaving on April 29, 2019 and will stay in the said
country until May 25,2019 and will be accompanied and taken care of by myself and
wife Margie Sillona;

4. That we are giving our full consent for the minor child to travel abroad with us;

5. That this affidavit was executed for the purpose of attesting to the truth of the
foregoing facts and for whatever legal purpose it may serve.
