sworn in accordance with law do hereby depose and state that:

1. My real name is Isidora Layba Tolentino;

2. I am the owner of Essie’s Beauty Parlor located at M.L. Tagarao St., Brgy.
Iyam, Lucena City;

3. It appear in the meralco billing for my business that my name is Essie

Tolentino;

4. I am executing this affidavit to attest that Essie Tolentino and Isidora

Layba Tolentino are one and the same person; and

5. I am executing this affidavit to affirm to the truthfulness and honesty of the

foregoing facts and for all intents and purposes it may serve.

appearing in the Meralco bill and the name "Isidora Layba Tolentino"
refer to one and the same person and that her true and correct name is
Isidora Layba Tolentino;

As such, we execute this Affidavit or attest to the truth of the foregoing

facts and for whatever legal purpose that this Affidavit may serve.
