This assessment chart will be used to collect anecdotal observational notes during the first day
of the learning segment. The teacher will write each student’s name in a separate square and

will be written in the far-‐left column where there is also additional room to leave any extra
comments about that particular student. As the teacher, I will then mark each square which
aligns with each objective, as the students completes each one.
