We acknowledge that in the event of any breach of documents submitted along with the Registration Form - 1(a) to
(f) on reverse of this form, and in the discretion of managers, coaches and Sports Authority of India, representative
swimmers may be:
(a) Deprived of privileges (b) Punished (c) Withdrawn from meet or camp
(d) Sent home immediately at his own expense (e) Suspended from SAI National Swimming Academy
Declaration:
1. I have gone through the rules and regulations for the membership of SAI National Swimming Academy at Dr.
Syama Prasad Mookerjee Swimming Pool Complex, New Delhi and agree to abide by those rules.
2. The information given above is correct to the best of my knowledge and if found wrong at any time, my membership
may be cancelled and the security deposit may be forfeited.
3. My ward will be participating in swimming practice and competitions on behalf of SAI.
4. I will not claim any compensation for his/her injury during swimming in the swimming pool. Management will not
be responsible for any injury or loss of life.
5. He/she will use life saving equipments and swimming costume as per the rules and advice of coaches/life guards.

Documents to be submitted along with the Registration Form:

1. Copy of the Birth Certificate duly attested by a Gazetted Officer.
2. Copy of the I D Card issued by the School, containing Admission No, age, Class and residential address.
3. Proof of Residence attested by the local Police Station.
4. Latest Medical Certificate issued by a MBBS doctor certifying that the applicant is medically fit for swimming and does not suffer
from any contagious disease.
5. Copy of the certificates of previous best performance self attested.

Note: The applicants are required to bring original documents at the time of registration.

ACADEMY RULES
1. When parents enroll a student at the Academy they agree to ensure that their child obeys the Academy rules set down
by the Management. These are intended to promote students’ welfare and to assist in the smooth functioning of the
Academy.
2. Trainees are expected to act in an orderly, courteous manner and to show consideration towards other people, e.g.
bullying, verbal abuse, and racism will not be tolerated.
3. Trainees are to be tidily dressed.
4. Trainees are required to attend all training sessions.
5. Trainees may not bring dangerous items such as knives, matches or explosives to the Academy.
6. Trainees are forbidden to bring cigarettes, tobacco, alcohol, drugs or solvents to the Academy or use such material
while under Academy discipline.
7. Coaches can confiscate items that disturb the class or are against Academy rules.
8. Cell phones are strongly discouraged at the Academy and if seen or heard during the training sessions, will be
confiscated. These are brought to Academy entirely at the students’ risk at all times.
9. Trainees are expected to obey the Academy rules.
10. The Academy may keep a trainee on detention. Parents will be notified for serious breaches of the Academy discipline/
damage to the property. Trainees may be suspended and later brought before the Board, who will decide their future at
the Academy.

CODE OF CONDUCT
1. In consideration of selection by the SAI Academy for Swimming, representative swimmers must:
(a) do as coaches direct them to;
(b) behave in a proper manner;
(c) notify coaches of any misbehavior by another trainee
(d) notify coaches of any medical conditions;
(e) not posses, consume, administer, purchase or dispose of drugs or alcohol;
(f) not behave in a manner which will bring the Sports Authority of India National Swimming Academy into disrepute;
