Incident & Accident Investigation Course (IAI)

Course Aims
To prepare supervisors and managers with the skills to determine actual and potential severity of
incidents, participate in investigation of a simulated incidents, use investigation techniques to identify
incident causation, create the ability to identify differences between immediate and underlying causes
and to provide the knowledge and understanding to develop recommendations to prevent recurrence of
an incident.

Course Objectives
On completion of the course attendees will be able to:
1. Explain the domino, multi-causal, HEMP and Tri-pod Beta models of incident causation.
2. Determine the actual and potential severity rating of an incident using the risk matrix.
3. Explain the criteria used to determine report ability.
4. Define the term ‘significant incident’.
5. Describe significant steps in carrying out an incident investigation
6. Participate in a simulated incident investigation.
7. Conduct interviews and confirm information for analysis
8. Demonstrate the ability to use barrier and control analysis and sequencing (incident tree) to
determine immediate and underlying causes of incidents.
9. Produce an incident report with ‘SMART’.
