DESCRIPTION : KATO crane observation before continue the activity;

Nearmiss 27 April 2019 caused by mazy wire rope at


crane drum. The wire rope went down without any
safeguard after the incident the wire rope was replaced.
The certificate shown from September 2018

Installed 3 sheaves on pulley block.
- No wire rope rolled at drum
(empty)
- No camera to drum monitor
- Rigger stopped, because the
operator couldn’t see the
drum was empty.
- There were no additional
winding on drum left and the
rope was not reaching the
ground

OBSERVER SUGGEST - Re-inpection the crane use third party.


- There should rest of wire (as per specification) at drum/winch.
- Wire rope length at specification is 175 meter, Need to
checked.
- In cabin, no camera to monitor at drum
- Check the len`gth and originality of wire rope
