JOINT AFFIDAVIT

That we, LIBRADA C. CAMPOY and MARIA C. TAAS, 82 and 76 years of age,
respectively, Filipinos, and residents of barangay Poblacion and Magastas, Sibulan,
Negros Oriental, respectively; under oath, do hereby depose and manifest:

That we personally know SANCHA CALINGACION EUMAGUE, who was born

on June 5, 1926 to spouses Manuel Eumague and Alejandra Calingacion;

That SANCHA C. EUMAGUE died last May 9, 2014 in barangay Magatas,

Sibulan, Negros Oriental;

That before she died she was a member of the COCOLIFE Insurance with a
Certificate of Group Life Insurance No. PCA: 00460, unfortunately her First Name in the
said Certificate of Group Life Insurance was erroneously entered as ZANCHA, instead
of SANCHA which is her true and correct First Name;

That the name SANCHA C. EUMAGUE, appearing in her Death Certificate and
the name ZANCHA EUMAGUE, appearing in her Certificate of Group Life Insurance
with the COCOLIFE, refers to just one and the same person;

That we know all these facts because we are good friends and neighbors in
barangay Magatas, Sibulan, Negros Oriental, for quite sometime before she died;

That we executed this affidavit freely and voluntarily without any considerations
or promise whatsoever; or to defraud any office of the government but to attest the
truthfulness and veracity of our statements herein.
