AFFIDAVIT OF LOSS

I, ZOSIMO D. UY, of legal age, Filipino, and with residence address at 54-
56 Roseville St., White Plains Subd., Quezon City, after having been duly sworn
to in accordance with law hereby depose and state:

1. That I am the owner/possessor of three (3) condominium titles with title

numbers and description are as follows:

a. Title No. -40378- Fourth Floor: - Unit 401 – Two Bedroom, One Living
Room, One Kitchen and One Bathroom with an area of 44.81 square
meters;

b. Title No. -40379- Fourth Floor: - Unit 402 – Two Bedroom, One Living
Room, One Kitchen and One Bathroom with an area of 45.80 square
meters; and

c. Title No. -40380- Fourth Floor Unit 403 - Two Bedroom, One Living
Room, One Kitchen and One Bathroom with an area 43.82 square
meters.

2. That the above condominium units are located at Murillo St., Quiapo,
Manila, covered by Transfer Certificate of Title number 189481/T-1214 of
the Registry for the City of Manila;

3. That on June 11, 2019, I was looking for the above-mentioned titles but to
no avail despite diligent efforts have exerted to locate the same, it appeared
that the same were inadvertently lost or might have been misplaced due to
unknown circumstances as the efforts to locate the same remain futile up
to the present and fear that the same might have been lost beyond recovery;

4. I certify and confirm that the above-mentioned condominium units were

never mortgaged, sold, conveyed, disposed of, or encumbered, to any other
person or entity. I certify and confirm further that said documents was
neither confiscated nor relinquished in favor of, any government entity
agency, or authority;

5. That I am executing this affidavit to attest to the truth of the foregoing and
for whatever legal purpose/s it may serve.
