A SEMI DETAILED LESSON PLAN IN HEALTH 10

I. OBJECTIVES
At the end of the lesson, the students are expected to:
a. Discusses the various forms of health service providers
b. To show appreciation and values acquired in perfoming a role play about health services
c. Demonstrate the role of health professionals, specialists, and health care services

II. SUBJECT MATTER:


TOPIC: Health Service Providers (1. health professionals’ 2. health facilities)
REFERENCE: Grade 10 Physical Education and Health Learner’s Module
pp. 204-207
MATERIALS: Visual Aids, video clip, projector, speaker

III. PROCEDURES:
A. Preliminary Activities:
1. Prayer
2. Greetings
3. Checking of attendance
4. Warm – up exercise

B. REVIEW
The teacher will be guided by the following questions:
1. What are the three components of consumer health?
2. What is health information?
3. What is health product?
4. What is a health service?
C. MOTIVATION
The teacher will be presenting a game called “Who wants to be a millionaire”

D. Lesson Proper
The teacher will discuss the following:
a. Various forms of health service providers
b. Health professionals, specialists and health care services
c.
E. APPLICATION
Now that the learners have knowledge about health professionals, specialist and
other health services.

F. GENERALIZATION
The teacher will be guided by the following questions:
1. What have you learned? How it is related in our own culture?
2. What are the important values that you learned from the lesson?

IV. EVALUATION
Each group will be given 10 minutes practice their own festival dance applying the
basic steps that they learned.

The learners’ performance will be assessed based on the following rubric.
