Rd. 5 Berm Nagtinig Floodway B Brgy. San Juan Taytay, Rizal, after being sworn to in
accordance with law, do hereby depose and state THAT:
1. I am the owner of a stall located at Taytay Municipal Public Market with the following
details:

Stall Number: B-A-B-1 #91

Stall Owner: Rosalyn Lebios

2. The said registered stall owner and I are the same as proven by my Certificate of Live
Birth (herein attached as Annex “A”);

3. I hereby waive, transfer and convey all of my rights, interests and obligations over said
Market stall in favor of LORELYN GAY S. ABALOS, likewise Filipino, of legal age,
and a resident of San Clemente Village I Pag-Asa Binangonan, Rizal;

4. I am executing this affidavit to attest to the truthfulness of the foregoing and in order
to facilitate the transfer of the said market stall in favor of Ms. Abalos.

DEED OF WAIVER OF RIGHTS

KNOW ALL MEN BY THESE PRESENTS:

This Deed of Waiver of Rights is entered into this _____ day of

the payment of the sum of Five Hundred Pesos (Php500.00), the FIRST PARTY
hereby assigns, transfers, and conveys, absolutely and unconditionally, all its rights, title,
and interests in the said stall, unto and in favor of the SECOND PARTY.
