WHEREAS, a complaint ( or information ) has been filed on 4th day of April, 2018 in
the above-entitled Court, Charging above mentioned Accused with the offense of “ACT OF
LASCIVIOUSNESS IN REL. TO Republic Act No. 7610” and she/he/they have been
admitted to bail in the sum of EIGHTY THOUSAND PESOS (Php 80,000.00)

NOW THEREFORE, I/We
