(see attached attendance)
It all started with introducing ourselves to parents, Ms. Rhea S. Bolatin as adviser and Mrs. Bernadette M. Montilla as my co-
adviser.

Agenda:

1. Election of Homeroom PTA Officers

2. School’s Rules and Regulations
3. Form 137 (for transferees)
4. PSA Birth Certificate

Proceedings:

1. Meeting called under School Memo#3 S. 2019 signed by our School Head, Mrs. Marites B. Jarina.
2. The following is the agreed new set of Homeroom PTA Officers for school year 2019-2020:
President : Bernadette Garcia
Vice President : Melanie Cauba
Secretary : Irene Orande
Treasurer : Rosalinda Olarte
Auditor : Mirasol Bardon
3. Mrs. Montilla reiterated the following school’s rules and regulations:

 Students’ tardiness and absences will be recorded by the class adviser so that in case the parents would want to know the
status of their children, there is a record to serve as basis. If the student fails to send a notice (excuse letter) to the teacher as
to the reason of his absence, so that the class adviser know the reason behind his absence. In addition, the teacher will
conduct home visits if cases like this occur.
 The poor quality of education is reflected in poor attendance, this will result to high dropped out and low achievement. She
also discussed that teachers and parents should work together for student success. It is not merely teachers who will motivate
the students to study hard, but it is also part of the responsibility of parents.
 Students should wear uniform everyday except Friday.
 On Friday, students will wear jogging pants and white T-shirt for their P.E. class.

4. Mrs. Montilla informed the parents of the transferees that the school will be the one to get the SF 10 or Permanent Record of
their children from their previous school. She also reminded those parents whose children have not submitted their PSA birth
certificate.
