My name is__________________ and I’m calling from the Fair Haven clinic. I work with the
Diabetes Prevention Program. The reason that I am calling is because you were referred by
your primary care provider to be tested for Diabetes. Did he/she talk to you about this?
If no: Sometimes the primary care provider will make the referral after the patient leaves. They
refer patients who are at risk for diabetes. Risk factors are:
What the Diabetes Prevention Program does is conduct group screenings about 3 times per
month to determine whether people are diabetic or at risk for developing diabetes. All patients
arrive between 8:45 and 9:00 in the morning fasting (no eating or drinking after midnight), have
their blood drawn, and then drink an orange drink called Glucola. Two hours later they have
their blood drawn again. While they wait we provide information on what diabetes is, how it
can be prevented, as well as nutrition and physical fitness education. At the end of the
screening, we provide lunch for all participants. The screening is totally free, whether you have
insurance or not, and runs until about 11:30am. I have a couple of upcoming dates:
