NOTES/ SPECIAL REQUESTS The Lessee would like to request for a Microwave Oven

The Lessor warrants that the unit will be professionally cleaned before
the move-in date.

The Lessor warrants that the entire electrical and plumbing systems have
been tested and are fully functional.

The Lessor ensures that the air conditioning system is serviced and
cooling properly before the move-in date, as well as the other electrical
appliances (refrigerator, TV, water heater, washer/dryer, etc.).

The Lessee ensures that all items turned over will be accounted for and
will be liable for any loss/damages.

SIGNING & EXECUTION OF LEASE The Lessor hereby agrees to hold and/or reserve the abovementioned

The Contract of Lease shall be signed and executed by the same parties on
or before the start of the lease period.

BROKER’S FEE The Lessor warrants that __________________ (Listing Broker) and
________________ (Leasing Broker) shall be entitled to a broker’s fee (50/50
sharing) equivalent to one (1) month rent for one year lease and for every
successful renewal.
