1.0 PURPOSE

GAMUDA BERHAD (GB) is the main contractor responsible for the


construction of Rumah Selangorku Cyber Valley Project hereinafter known
as RSKUSCV, at Cyberjaya Selangor.

Gamuda is committed and given top priority to ensure the safety & health of all
workers and visitors at RSKUSCV site. The purpose of this Emergency
Response Plan is to describe the arrangements for coordination and control of
any incident that requires an immediate response to preserve life or human
wellbeing, protect property and prevent any escalation of the effects of any such
incident.

2.0 SCOPE

This Plan details the process of planning, implementing and evaluating


measures taken to reduce the risks to a tolerable level in the event of any
emergency at Project Site. The Emergency Plan does not cover the
arrangements for crisis management but may form part of the crisis
management arrangement.

The plan shall be updated and maintained to reflect any changes and
improvement required.

OBJECTIVES

The Objectives of the Plan are;

 To prevent fatalities and injuries and reducing damage to property and


environment.
 To establish and implement a formalized mechanism to effectively manage,
control, response and contain the emergency situations.
 To comply with the legal and other requirements.
 To accelerate the resumption of normal business operation.

3.0 DEFINITION

Emergency Any unplanned event, which has the potential to cause


injury to people, damage to property and environment
and loss to production or combination of all the above,
which requires immediate action to control the situation.

Emergency Trained personnel on basic emergency required on


Response Team evacuation of personnel from work areas in an
(ERT) emergency situation and assist relevant authorities when
required.

Assembly Area Refers to a designated and safe location for people to


assemble during an emergency.

First Responder Any person who first detects any emergency or


dangerous situation.

Major Fire Fire that may cause serious injuries, loss of life, extensive
damage to property or serious work disruption and
required assistance from external emergency services.

Minor Fire Fire that localized and can be easily extinguished with
existing resources and facilities.

Debriefing A short meeting lead by the Emergency Commander after


the termination of emergency.

Termination of End of an emergency and declaration that it is safe for


Emergency people to return to their normal work.

4.0 RESPONSIBILITIES

4.1 ROLES AND RESPONSIBILITIES

Project Site has established its own Emergency Response Team and headed
by an Emergency Commander.

The table below shows the roles and responsibilities of ERT members

emergency and proposed corrective actions to prevent
any reoccurrence.
• To assume the position of Emergency Commander in
his/her absence.
• Keep personal log and record significant events.
3 Warden/ • To raise the alarm in the event of an emergency and

from Security/Logistic for the purpose of conducting
headcount

• Control the crowd and conduct headcounts at the


Assembly Area and ensure the safety and security of
the workers.
• To ensure injured person is being attended by the first
aider.
• Assist in the Incident Investigation
• To inspect firefighting facilities at his/her respective
responsible area and report any malfunction or
rectification required.
4 First Aider • To plan and organize a safe first aid treatment center at
assembly area and ensure all facilities are well equipped
• To arrange without any delay medical assistance (i.e.
ambulance & etc.) unless injury is so minor and it can be
handled without professional attention.
• To promptly provide first aid treatment to the injured
person before sending the casualty to hospital/clinic
during an emergency.
• Assist medical emergencies (ambulance) on their arrival
and update the Person-in-charge with relevant details.
• Stabilize the condition of victim.
• To assist in investigation, if required to do so.
• Keep personal log and record significant events.
5 Security & • To assess the situation if alarm is triggered and liaise
logistic with respective warden to verify whether it’s a false alarm
or real emergency and updates the Emergency
Commander.
• To update the Wardens on the no of visitors,
subcontractors and client at the premises during the
event of an emergency.
• To guide workers to assembly area.
• To ensure vehicle movement is stopped during an
emergency.
• To barricade or cordon the affected area and only allow
assigned personnel and emergency services to enter the
site.
• To secure the premises from any break-in or vandalism
during the commotion.
• Control the traffic and ensure the assembly area is in
safe condition.

• Provide guide and assist the external emergency


services on logistic matters and ensure clear access and
parking facilities provided for emergency vehicles.
• To report any malfunction or damaged firefighting
facilities during the regular patrolling.
• Assist in the Incident Investigation if required to do so.
• Keep personal log and record significant events.
• Assist in firefighting or search and rescue operation as
instructed by the Emergency Commander.
• To maintain record of events prior to emergency.
• To maintain a chronology record of the emergency.
• To collate and consolidate report from all controllers and
set up master event log for recording all significant
events
6 Fire Fighting • As a part of the Emergency Team directly responsible for
early response to fire emergencies at the site. Provides
support if needed for BOMBA when they arrive.
• Advice BOMBA, on the source of water at fire scene
• Fire fighting teams are also First-Aiders; they can help to
casualties in case of emergencies.
• Inspect firefighting equipment and keep record
7 Search & • Assist BOMBA when necessary by rescuing

cars, etc.

8 Workers/ • Learn the site’s evacuation plan, know the escape route.

during evacuation. Evacuate only upon being instructed.
• Evacuate the site promptly in an orderly manner and
using the nearest and safe fire exits.
• Report to designated assembly area and await
instructions from the emergency commander/ deputy
emergency commander.

5.0 PROCEDURE

5.1 TYPES OF POTENTIAL EMERGENCIES

The following potential incidents that may require an emergency response have
been identified;

i) Fire and Explosion


ii) Fatality
iii) Serious injury
iv) Bomb threat or suspicious package
v) Riot, demonstration or civil disturbance
vi) Natural disaster – earthquake , landslide, adverse weather condition, major
flooding
vii) Structural Collapse
viii) Road traffic incident
ix) Major hazardous chemical spillages/ Environmental pollution
x) Unexploded ordinance

5.2 TYPES OF EMERGENCY LEVEL

i) Level One (1)

• A minor, localized incident that can be easily resolved with existing


resources.
• Has minimum impact on personnel or normal operation.

• Require activation of Emergency Response Team and may not require


activation of emergency evacuation or external emergency services.

ii) Level Two (2)

• A medium level emergency that disrupt the operation at Project Site only.
• May have serious consequences and impact life safety.
• Require localized activation of emergency evacuation and assistance from
external emergency services.

iii) Level Three (3)

• Major incident that adversely affect the entire project site and may affect
the surrounding community.
• Normal operation could be suspended and the impact is wide-ranging and
complex.
• Require broad cooperation and extensive coordination within Project Site
ERT and external emergency services.
• Require activation of emergency evacuation for entire project site and
assistance from external emergency services.

5.3 EMERGENCY RESPONSE TEAM (ERT)

The selection of Emergency Response Team members are based on the level
of authority, capability and competency. The actual composition of ERT shall
be decided based on the capacity of the occupant, nature of the business and
the risk level. The Emergency Response Team (ERT) is responsible to;

 Providing immediate response during an emergency either to terminate or


rescue or, if possible, contain the situation until the arrival of external
emergency services or assistance.

 Assisting external emergency services by providing them appropriate


information on the condition of the emergency and acting as the focal point
on the planning, preparedness, protection and response during an
emergency.

 Imparting training to maintain a high degree of preparedness, conducting


drills and ensuring the effectiveness of the Emergency Response Plan.

Emergency Response Team Organization Chart is as in Appendix A and


subject to be updated according to the changes and requirements.

5.4 EMERGENCY CONTACT

Emergency contact number of ERT members, external emergency services,


relevant local authorities and emergency hotline number are communicated
and displayed at prominent places at the workplace.

Emergency Contact for Project Site is as sample attached in Appendix B and


will be updated when and as necessary.

5.5 EMERGENCY EQUIPMENT AND FACILITIES

• Project Site is equipped with the following fire protection system ;


i) Fire alarm (loud hailer)
ii) Fire Extinguisher (ABC and CO2)
iii) Emergency Light
iv) Emergency Exit Signage

• Emergency response facilities and equipment are listed as in Appendix C


and subject to be updated as an when necessary which include but not
limited to the following items;

i) Fire blanket
ii) First aid facilities (1st aid box, etc)
iii) Stretcher
iv) Torch light
v) Emergency safety signage and barricades
vi) Traffic control equipment such as delineator, barrier, cone, etc
vii) Communication devices such as walkie-talkie, loud hailer

• The emergency protection system and response facilities will be inspected


on regular basis and during SHE Inspection to ensure the facilities are in
good condition.

• Periodical testing will be carried out to ensure the fire protection facilities are
in good condition and functioning in the event of any emergency.

5.6 EVACUATION PLAN AND ASSEMBLY AREA

• Evacuation plan and the location of Assembly Area are prominently


displayed at site and communicated to all workers and visitors.

• The evacuation plan (sample as Appendix D) contains the nearest exit route
and the emergency contact details.

• The emergency exit shall be;


i) Clear from any obstruction that may delay or interrupt the evacuation
process.
ii) The nearest and fastest way for workers to evacuate the site in a
safe manner.
iii) Easy to access by authority.

5.7 BASIC EMERGENCY PROCEDURE

a. Person (identifier) who detects any emergency or unsafe situation shall


inform the immediate superior and respective warden.
b. The person (identifier) / warden shall trigger an alarm using loud hailer or
shouting for help.
c. When the alarm triggered the Emergency Commander activated the
Emergency Response Team to response on the scenario
i) Level 1
• Can be controlled and managed internally. Requires the activation of
ERT only.

• No emergency evacuation and external emergency services is


required.

• Once the emergency has been rectified, announcement will be made


to update the workers.

• Project Site Emergency Commander shall ensure the situation is safe


to continue with normal business operation.

• ERT will conduct incident investigation to prevent any recurrence.

• Security Officer shall update the person in-charged

ii) Level 2
• ERT commander shall activate the emergency evacuation and
instruct the Emergency deputy commander to call BOMBA or other
relevant emergency services.

• ERT Commander shall declare the emergency situation and


announcement will be made for the activation of evacuation process.

• ERT Commander shall instruct the ERT members to take charge of


their respective roles and responsibilities.

• ERT Commander shall inform the Top Management on the


emergency situation and update the status.

• Warden/ Evacuation shall instruct the workers to proceed to the


assembly area using the nearest and safe emergency exit. Visitor
and Immobility-impaired persons (if any) shall be given priority and
assisted during the evacuation.
• Warden/ Evacuation shall ensure all workers have evacuated from
the work area, and stop all job before proceed to the assembly area.
• Security & logistic shall implement all measures to secure premises
from any break-in or vandalism take place during the commotion.
• Security & logistic shall arrange the logistic for the arrival of
emergency service vehicles and control the traffic at the assembly
area to prevent any risk to the workers.
• Warden to carry out headcount of workers of their respective area
and report on headcount, missing or injured personnel.
• ERT Deputy Commander shall get the update of the head count,
missing and injured person from Roll caller/ Warden
• ERT Deputy Commander shall update ERT Commander on the
headcount and information on any missing or injured personnel.
• ERT Commander shall activate the search and rescue operation if
necessary and update on the status.
• Upon arrival of emergency services, ERT Commander shall report
and hand over the duty to the BOMBA Officer or other relevant
authority on- duty.

• ERT commander shall instruct the respective ERT member to


guide and assist the emergency services if necessary.
• Security & logistic shall record the whole chronology of the
emergency and observe the situation.

• Upon completion of inspection and rescue operation, the external


emergency services shall update ERT Commander on the overall
status.
• ERT Commander shall announce emergency termination upon
confirmation/approval received from the BOMBA officer or other
relevant authority on- duty.
• ERT Commander will conduct de-briefing session within ERT
members and instruct incident investigation to be conducted if
necessary.
iii) Level 3

• Emergency evacuation shall be activated and the overall


evacuation and response procedure will be led by Emergency ERT
Commander.
• The evacuation procedure shall follow the same as emergency
level 2

5.8 POTENTIAL EMERGENCIES

a) Fire and Emergency

Minor or Controllable fire


• Fire that localized and can be easily extinguished with existing resource and
facilities.

Major or Uncontrollable fire


• Fire that may cause serious injuries, loss of life, extensive damage to
property or serious work disruption and required assistance from external
emergency services.

• In the event of fire, evacuation priorities will be from the area on which
emergency has occurred.

b) Trapped in the smoke and fire

• If there is a phone in the room, call the ERT members or BOMBA and tell
exactly where you are.

• Stay low. Smoke rises, and you will find the cleanest air near the floor.
Cover your mouth with a damp cloth to breathe through.

• Don’t be panic and be patient.

c) Bomb Threat or Suspicious Package

• Although most bomb threats are prank, every bomb threat must be considered
as genuine.

• The decision to move or evacuate the workers will be determined by police,


bomb squad or fire department authorities. This will be a complex decision
based on many variables.

• Speed in reporting and preplanning is absolutely critical especially when


involving recognized authorities and determining “chain-of-command”.

• When receiving a bomb threat call by phone, try to secure as much as


information as possible and pay full attention. While on the phone, give
instruction to someone to call the Emergency Commander.

• If possible, try to secure all the information required in the Bomb Threat
Checklist

• Emergency Commander shall inform the Police department upon instructed by


the Emergency Commander.

• The emergency commander will ensure that the top management are informed
of the incident.

• The decision to evacuate the site is generally determined by specific


information received regarding the bomb threat and instruction given by police
(bomb squad).

• The Emergency Commander will initiate the evacuation as deemed appropriate


based on the available information and instruction given by Police.

• The evacuation process will follow the evacuation process for fire. No one will
be allowed to re-enter the site until the bomb squad completely inspected the
site and authorized the Emergency Commander to terminate the emergency.

• The process will be the same if anyone found suspicious packages. Do not
open the package and immediately informed the Emergency Commander for
further action.

d) Major Chemical Spillage


• Remain calm and reassure others safety.

• Evacuate affective the area

• Call 999 and give details of the accident including location, types of
hazardous materials involved, and whether there is personal injury.

• If the accident involves personal injury or chemical contamination, follow the


below steps as appropriate and at the same time:

• Move the victim from the immediate area of spill (if this can be done without
further injury to the victim or you).

• Locate nearest emergency eyewash or safety shower. Remove any


contaminated clothing from the victim and flush all areas of the body
contacted by chemicals with copious amounts of water for 15 minutes.

• Administer first aid as appropriate and seek medical attention.

5.9 TERMINATION OF EMERGENCY

As soon as the emergency is rendered over, it is essential to ensure the safety


and integrity of the work place before the emergency is terminated.

The Emergency Commander will declare “ALL CLEAR “once the emergency is
terminated for normal operations to resume.

The following criteria must be met before declaring termination of emergency:

 The work place has been re-instated as in normal condition.


 All rescue equipment and facilities removed from the work place.

The relevant personnel at the relevant function shall reset the machines,
emergency equipment/system to normal operation. Inspection, repair and
replacement of any faulty equipment shall be done thoroughly. Person in charge
of respective department shall submit damage report c/w action plan to the
Emergency Commander within 7 working days upon termination of emergency.

5.10 DEBRIEFING AND POST MORTEM SESSION

A debriefing session will be conducted after the emergency has been


terminated. The ERT members and external emergency services (if necessary)
are required to attend the debriefing session.

The briefing will be chaired by Project Site Emergency Commander to address


the following issues:

 Chronological developments of the event (identify / state how the


emergency started, if possible).
 Actions taken during the emergency as outlined in the emergency procedure
and any additional actions taken to control the incident.
 Safety measures to be taken after the termination of the emergency.
 Reports on the number of injuries as the result of the emergency.
 Follow-up activities.
 Feedback from ERT Members.

 Recommendations to prevent future recurrence or to improve emergency


response actions.
 Selection of the Investigation Team (if necessary) and preparation of report.
 Changes to be made in the existing Emergency Response Plan and
HIAROC.

5.11 MEDIA AND PUBLIC RESPONSE

Certain type or degree of emergency may catch the attention of public, mass
media and 3rd parties. For any emergency situations which involve public and
media, the Emergency Commander shall;

• Ensure no media personnel enter the premise without consent from the ERT
Emergency commander.
• Ensure that there are no personnel or workers who provide any comments
/ statement to the media without consent from the Emergency commander.
• Ensure that Group Public Affair (GPA) personnel are assigned on site to
handle the logistical impact of any media presence.

• Consult with GPA before giving any statement to media that may jeopardize
the integrity of the organization.
• Consult with GPA and/or HR before communicating with the family if there
is any situation where family members are aware of such incident and may
be present at the scene or in the event of any injury/fatality.
Note: GPA shall keep the stakeholders well informed on the situation i.e. with
the advice from the Emergency Commander

5.12 TRAFFIC AND CROWD CONTROL

Site SHE team shall coordinate with external agencies to ensure adequate and
safe control.

PROJECT SITE Security shall not allow any party to enter into premise without
consent from the Emergency Commander

5.13 TRAINING

Training on emergency management and control are essential to ensure all the
ERT members understand on their pertinent roles and responsibilities and able
to response effectively in the event of any emergency.

Training programs shall provide the ERT members with applicable knowledge
on handling an emergency and built confidence required when performing
actual emergency.

The Emergency Commander shall be responsible to identify the training needs


relating to the foreseen potential emergencies and if possible with the
collaboration of external emergency services.

The training on Emergency Preparedness and Response may include but not
limited to the following items;
• First Aid
• Cardiopulmonary Resuscitation (CPR)
• Emergency Management
• Fire Fighting

• Search & rescue


• Emergency Drill

Information on ERP, evacuation procedure and roles and responsibilities of
occupant shall be communicated through SHE Induction, awareness training,
talk and emails.

5.14 EMERGENCY DRILL

Emergency exercise or drills are important tool in keeping ERP plan functionally
up-to-date. The exercise shall be conducted at least once a year or as when
deem necessary. This regular exercise is to confirm the effectiveness of the
Emergency Response Plan and any necessary improvements required so as
to maintain the readiness level should an emergency occur.

The exercise could ensure that equipment is deployed, communication line is


tested, local emergencies services response is tested and the levels of ERT
members’ preparedness are evaluated.

The objectives of the drill are detail as following;

• To comply with legal and other requirements.


• To provide opportunities to test the equipment, communication flow and
response of local emergency services.
• To monitor and analyze the response of occupant during actual emergency
situation.
• To review the preparedness and effectiveness of ERT members and
prepare them to handle an actual emergency situation.
• To assess the needs of additional resources or equipment.

The result of the drill shall be properly documented and communicated to the
workers.

5.15 REVIEW

The result from any emergency or emergency exercise will be evaluated to


identify necessary changes or mitigation measure to improve the existing
Emergency Response Plan (ERP).

The ERP shall be reviewed periodically where necessary to ensure


effectiveness of the plan, arrangement, equipment, training and the
preparedness level of all the personnel.

The Emergency Commander shall ensure that the revised ERP be


implemented and communicated to all staff.

5.16 INCIDENT INVESTIGATION AND REPORTING

Any emergency event that requires incident investigation to be carried out, the
assigned ERT member shall follow the Incident Investigation and Reporting
Procedure referenced KJHP1-MSP-QSHE-0012.

5.17 BUSINESS CONTINUITY PLANNING (BCP)


A business continuity plan is a plan to continue operations if the site is affected
by different levels of disaster which can be localized short term disasters, to
days long building wide problems, to a permanent loss of a work area.
Procedure for BCP
• In the event of a site evacuation, all staff assembles at the emergency
evacuation Area.
• Emergency response team proceed to the designated meeting point
after normal evacuation procedures
• Emergency commander determine if medical assistance is required and
organize
• Reassure staff and remind them not to talk to the media, particularly
when the cause and impact is unknown.
• Keep open (i.e. limit use of) phone number was advised and await
instructions
• Confirm recovery strategy, schedule, procedures and resourcing.
• Remain on alert until crisis is resolve.
