 The schedule of activities
 The requirements in the submission of documents by the bidder/s

Having no other issues and concerns to be discussed, the meeting was

adjourned at 4:00 o’ clock in the afternoon.

Dear Engr. Mendoza:

As a result of the Post-Qualification conducted by the BPSU Technical Working Group for
the CONSTRUCTION OF COLLEGE OF BUSINESS AND ACCOUNTANCY BUILDING-
BPSU MAIN CAMPUS with PhilGEPS Reference No. 2015020031849, it was found out
that the Bid Proposal you submitted during the Opening of Bids held on January 14, 2016
was the Lowest Calculated and Responsive Bid.

As such, please submit three (3) copies of your latest Income and Business Tax Returns
filed and paid through BIR Electronic Filing and Payment System (eFPS) within a non-
extendible period of three (3) calendar days from the receipt of this Notice.

Failure to submit the requirements within the prescribed period or any finding against
the veracity of such shall be a ground for forfeiture of your bid security and your
disqualification for the award of the project.

Greetings!

The Government Procurement Reform Act (RA 9184) Section 13. States that “To
enhance transparency of the process, the BAC shall in all stages of the procurement
process, invite, in addition to the representatives of the COA, at least two observers to sit
in its proceedings, one from a duly-recognized private group in a sector or a discipline
relevant to the procurement at hand, and the other from the non-government
organization.

With the above legal basis, the university would like to invite you to participate, as
one of the observers, in the conduct of the procurement activities of the Bataan Peninsula
State University (BPSU) for the Construction of the College of Business and Accountancy
(CBA) Building at BPSU Balanga Campus to be held on January 14, 2016, 2nd Flr,
Administration Bldg., BPSU Main Campus, City of Balanga, Bataan.
