B. FAMILY BACKGROUND
“Mrs. Lady” is a 42 years old woman, residing at Purok Santos, Barangay
Kipalbig, Tampakan, South Cotabato, Philippines. She is 25 years married to “Mr.
Senior", a 50 years old Tricycle driver. Living in nuclear type of family, “Mrs. Lady”
lived to be a loving housewife and a caring mother to her five (5) children namely:
“Troy”, “Gabriella”, “Ryan”, “Sharpay” and “Chad”. Troy being the eldest child in
the family supports most of the financial needs of the family since he works as a
public school teacher in Tampakan Elementary School. “Gabriella”, the child next to
“Troy” helps in financing their needs as a family by becoming a Sales Girl in KCC
Mall of Marbel. “Ryan” and “Sharpay” are working students and both studying at
Notre Dame of Marbel University. While “Chad”, the youngest sibling didn’t continue
his study to college due to financial instability and decided to help his mother in the
daily household tasks. “Mr. Senior”, who works as a Tricycle driver, rounds the
street of Tampakan 5 times every week with his rented Tricycle owned by his friend.

The family earns 20 thousand more or less in a month and as the “Ilaw ng
Tahanan”, “Mrs. Lady” is the one who budgets the money for expenses (including
electricity, tuition, food and others). The family eats three (3) times every day
usually composed of rice and egg or fish at breakfast; rice and vegetables at lunch;
and rice and fish or vegetable at supper or sometimes they purchase meat for food
depending on “Mr. Senior’s” daily collection. The family also likes to eat fruits that
are available in season. “Mrs. Lady” likes to eat salty as well as meaty foods but is
not engaged into vices. Unlike his wife, “Mr. Senior” drinks alcoholic beverages
sometimes in a week. Other than those lifestyle, the rest of the family members
doesn’t engaged into smoking or drinking alcohol.

The family is believer of the Church of Christ and always attending the
weekly worship. Some of the family members are engaged in some organization in
their community like “Mrs. Lady” who was elected as one of the officers of Purok
Santos; “Mr. Senior” who is a member of Tampakan TODA (TAMTODA) and “Troy”
who presently applying his membership in the Philippine Public Teachers
Association.

About the medical history, the family members have no known disease
except for “Mr. Senior” and “Mrs. Lady” who have hypertension for 10 years and
more. Considering the fact that “Mrs. Lady” is 42 years old and hypertensive, she
got pregnant again on her 6th child, 17 years after her last pregnancy.
She consulted the Barangay Health Center twice every month for medical checkup.
She was advised to limit her salt and fat intake into minimum. Due to pregnancy
and hypertension, she was permitted to do heavy household chores and advised to
take a rest. So, all of the household chores are held over to “Chad” until “Mrs. Lady”
delivers.
C. HISTORY OF PAST ILLNESS

“Mrs. Lady” doesn’t have any childhood illness or hospitalization during her
childhood years. She doesn’t experience any problem such as cold or fever for the
past six (6) months except to her elevated blood pressure which is present 10 years
ago. But during her first (1st) trimester, of pregnancy, she experienced nausea and
vomiting. Her blood pressure elevated from 130/90 mmHg to 140/100 mmHg as
compared by the Barangay Health Worker.

D. HISTORY OF PRESENT ILLNESS

“Mrs. Lady” is a 42 years old woman who is suffering from hypertension for
the past 10 years. She consulted a medical doctor for treatment but failed to
continue the treatment due to financial instability. Due to this, she tried to use
herbal plant to control her blood pressure. She chews fresh leaf of Gutokola and
extracts the liquid on it. She believed that this treatment will control her blood
pressure. She stopped taking this medication when she knew that she is pregnant
on her sixth (6th) child to prevent any effects on her developing child.

Three (3) days prior to admission, “Mrs. Lady” experienced mild lower
abdominal pain and noticed that she has spotting. She didn’t bother those sign and
symptom experienced because she have experienced those on her previous
pregnancies and that she can handle it considering that this is her sixth (6th)
pregnancy.

Three (3) days after, she was rushed to South Cotabato Provincial Hospital
due to severe pain and elevated blood pressure. She was then admitted in the
Obstetric Ward then to the Delivery Room. She delivered her baby dead due to fetal
distress. According to “Mr. Senior”, the incidence happened three (3) days prior to
admission has a connection to the death of his baby. Excessive bleeding occurs
after the delivery of the placenta due to the relaxation of the uterine muscle. “Mrs.
Lady” was immediately sent to the operating room for STAT Subtotal Hysterectomy
to control the bleeding under the service of Dr. Joy L. Vencer and Dr. Alfredo Aguila.
The surgery lasted for 3 hours and 20 minutes. She was then sent to the Recovery
Room for monitoring and evaluation. “Mrs. Lady” was diagnosed of having Uterine
Atony.
E. EFFECTS OF HOSPITALIZATION

To the patient:

The effect of hospitalization to the patient is immense because she delivered


her baby dead. She also undergone STAT Subtotal Hysterectomy due to Uterine
Atony and Hypovolemic Shock and stayed in the Recovery room for 24 hours. There
the patient is inserted with ENT tube to aid in her ventilation.

To the family:

The effect of the hospitalization in the family is huge also because they didn’t
expect that the baby will not survive the intra uterine life and that “Mrs. Lady”
undergone an immediate operation without any financial preparation. The family is
also restless due to left and right request o blood products needed to be given to
“Mrs. Lady” in the OR to sustain her life.

F. EXPECTATION DURING HOSPITALIZATION

From the Family:

The family expects that all possible interventions and treatment will be given
to “Mrs. Lady” appropriately. They also expect that the operation will be successful
and that “Mrs. Lady” will recovery fast without complication and also to recovery
from her loss as a mother.
