Premorbid Personality

Our patient Elsie Sabate is a 53 y.o. woman, single and a resident of


Tibungco, Davao City. She lives with her cousin. As our patient narrated, she
said that her father is already dead while her mother is still alive and presently
lives in Cotabato City. She has no siblings. She had her first boyfriend when she
was 15y.o. and she got pregnant at the age of 17. The father of her daughter did
not take his responsibility for the child. Because of poverty, Elsie’s mother was
prompted to let the child be adopted with whom she doesn’t know. Since she was
still a high school student at that time and don’t have any idea on how she will
going to sustain the needs of her daughter, she then agreed with her mother’s
idea and left her no choice.
She also narrated that she had been to Japan. She met a Japanese man
here in the Philippines and invited her to work as a nanny for his mother there in
Japan. However, after a month of stay in Japan, she said that she was confined
by that Japanese man to a mental hospital there in Japan since she was caught
loitering around the city without knowing where she’s headed to. And so not long
enough she was brought back in the Philippines.
Elsie worked as “masahista”
In conjunction with her depressive symptoms, Elsie is also having anxiety.
Her anxiety is associated with restlessness, tiredness and insomnia. Elsie
express concern over her lost child, due sometime whenever we bring out the
topic about her daughter she will suddenly pause and go to a flat affect state but
would go back to euthymic mood once the topic is changed.

Present Illness

Three years ago our patient Elsie was brought by a police officer and was
brought back to the psychiatric hospital. She said, she was loitering around the
city and for three days without food. During this time, she claimed to have no
recollection as to where she was and why she was loitering, until she met a
police officer whom she claimed as her friend. She then requested, the officer to
bring her to the psychiatric hospital.

Past Illness

a. Psychiatric Illness
Elsie Sabate has been confined with psychiatric problems for three times.
Her first admission…. Her second admission... And finally, three years ago.
b. Non-Psychiatric Illness

Personal And Social History


a. Infancy
Elsie is the only sibling. None among her parents had a history of
psychiatric disorder.

b. Preschool
As far as Elsie could remember her mom use to discipline her with
excessive physical contact. She stated that her mom treated her as if she
was not her daughter. There is a great gap between her relationships with
her mother.

c. Adolescence
Elsie reached up to 2nd year high school, in the University of
Mindanao Davao City, however she was not able to finish the school year
because she got pregnant.

d. Adulthood
Social
Elsie worked as “Masahista”. She use to be a smoker and an
alcoholic drinker. She said that she is a loner type of person who is
not fond of making friends.

Vocational
She was not able to finish any degree, not even high school.
She just reached until 2nd year high school. She also stated that she
finds it very tiring to go to school & study.

Sexual
Elsie had her first boyfriend when she was 15 years old. It
was then she had her first kiss, as she narrated. She got pregnant
at the age of 17. She also said that she had a live-in partner who
unfortunately left her and married other woman.
